怀柔区流感住院病例特征及其住院率估计

Study of characteristics and estimation of hospitalization rate for hospitalized cases of influenza in Huairou district

摘要:

目的 分析怀柔区2017-2018年严重急性呼吸道感染(severe acute respiratory illness,SARI)病例流行病学特征,并估算流感导致的SARI住院率.方法 在北京市怀柔区的SARI哨点监测医院选择2017年第14周至2018年第13周筛查登记的SARI病例作为调查对象,收集其性别、年龄、临床病程及慢性病患病情况,并采集病例咽拭子标本进行流感病毒核酸检测,计算SARI病例住院率及流感导致的SARI住院率.结果 2017-2018年怀柔区共纳入SARI病例724例,采集咽拭子标本657份,其中,流感病毒阳性率为20.4%(134/657),134例流感确诊SARI病例组年龄中位数(P25,P75)为5(3,58)岁,低于非流感组[24(3,62)岁](Z=-2.24,P=0.032);流感确诊SARI病例患慢性支气管炎的比例[6.0% (8/134)]高于非流感组[1.2% (6/523)](x2=9.70,P=0.002).2017-2018年估算的全人群SARI住院率和实验室确诊流感住院率分别为269/10万和55/10万;0~1岁年龄组SARI病例住院率最高,为2 317/10万,2~4岁年龄组实验室确诊流感住院率最高,为529/10万.新甲型H1N1[A (HINI) pdm2009]、季节性甲型H3N2[A (H3N2)]和乙型Yamagata系(BY)流感病毒所致住院率分别为23/10万、13/10万和18/10万.结论 患有慢性支气管炎人群是罹患流感的高危人群,2~4岁组儿童的流感住院负担较重.

Objective To identify epidemiological characteristics of Severe Acute Respiratory Illness (SARI) cases and to estimate the hospitalization rates of the SARI cases attributable to influenza in Huairou district of Beijing,2017-2018.Methods SARI surveillance were conducted at BeijingHuairou Hospital from the fourteenth weeks of 2017 to the thirteenth weeks of 2018.Inpatients meeting the definition of SARI were enrolled,and the information of gender,age,medical history and disease outcomes was collected.Respiratory tract specimens were also collected for influenza pathogen detection.The hospitalization rates of SARI and the hospitalization rates attributable to influenza were estimated.Results A total of 724 SARI cases were enrolled,and respiratory tract specimens were collected from 657 cases and 20.4% of the samples were positive for influenza virus.The incidence of chronic bronchitis in influenza group (6.0%,8/134) was higher than that of non-influenza cases (1.2%,6/523) (x2=9.70,P=0.002).The median age (P25,P75) of 134 influenza confirmed SARI cases was 5 (3,58) years old,and was lower than 24 (3,62) years old in non-influenza group (Z=-2.24,P=0.032).The hospitalization rates of SARI during 2017-2018 was 269/100 000 with the highest rate among 0-1 year old children (2 317/100 000).Influenza was associated with an estimated 55 SARI hospitalizations per 100 000 with the highest rate among 2-4 years children (529/100 000).The hospitalization rates attributable to influenza virus A (H1N1) pdm2009,A (H3N2),and BY were 23/100 000,13/100 000 and 18/100 000,respectively.Conlusions People with chronic bronchitis were at higher risk for influenza.The hospitalization burden of SARI attributed to influenza was the greatest in 2-4 years old group.
