摘要目的 探讨肺部恶性肿瘤患者在螺旋CT引导下经皮肺穿刺行射频消融(RFA)治疗的护理.方法 总结分析13例不能实施手术治疗的原发性肺癌和3例肺转移癌,共21次RFA治疗的术前准备、术中配合以及术后病情观察等全程护理工作.结果 21例次RFA治疗全部成功.术中并发气胸2例,咳嗽,痰中带血7例次,胸痛5例次;术后有11例次患者体温达37.5～39.5℃,肺内感染1例,气胸合并胸腔积液1例.这些并发症经控制感染、胸腔穿刺排气、排液等对症治疗以及加强护理,患者均在短时间内治愈.结论 加强术前患者心理教育和围手术期护理,可预防和减少RFA治疗过程中并发症的出现,一旦出现并发症要尽早发现、及时处理,提高RFA的治疗效果.

abstractsObjective To evaluate the nursing methods of percutaneous Radio-frequency ablation (RFA) of lung tumors under CT-guidance. Methods There were 21 treatments of RFA on 16 patients including lung cancers (13 eases) and lung metastases (3 cases). All patients received health education and psychological nursing before operation, careful co-operating during operation and corresponding observation after operation. Results The successful rate of RFA was 100%. However, during RFA there were 2 cases appeared the symptom of pneumothorax, 7 cases appeared the symptom of cough and hemoptysis and 5 cases appeared the symptom of chest pain. After RFA, 11 cases appeared the symptom of fever from 37.5 ℃ to 39.5 ℃, 1 case appeared the symptom of pneumothorax complicated with pleural fluid and 1 case appeared symptom of pulmonary infection. These complications disappeared through corresponding treatment and enhancing nursing care. Conclusions Intensive psychological nursing and nursing during perioperative period are very important for prevention and decreasing the incidence of complication of RFA. It is necessary to early detection and dealing with complication to improve the therapy effect of RFA.
