目的 描述肺结核患者病耻感现状,并分析肺结核患者病耻感与治疗依从性之间的关系.方法 采用便利抽样法,选取2015年8月—2016年2月在北京市胸科医院就诊、符合纳入标准的120例肺结核患者进行问卷调查.使用肺结核患者病耻感量表(TSS)测评患者的病耻感,采用Morisky服药依从性量表(MMAS 8-item version)测评患者的治疗依从性.结果 患者他人贬低维度病耻感得分为(27.71±9.48)分,自我贬低维度病耻感得分为(25.73±8.65)分.患者自我贬低维度病耻感得分与治疗依从性呈负相关(r=-0.194,P=0.03).结论 护理人员应当加强患者的心理护理,减轻其病耻感.同时有针对性地从不同角度采取护理措施,提高患者治疗依从性.

Objective To describe the status quo of stigma in patients with pulmonary tuberculosis (TB) and to analyze the relationship between their stigma and treatment adherence. Methods Totally 120 TB patients who were treated in Beijing Chest Hospital between August 2015 and February 2016 and met the inclusion criteria were selected by convenient sampling and investigated with questionnaires. The tuberculosis-related stigma scale (TSS) and Morisky Medication Adherence Scale (MMAS 8-item version) were used to assess the patients' stigma and treatment adherence, respectively. Results The patients' stigma score in the dimension of other's depreciation was (27.71±9.48), while their stigma score in the dimension of self-depreciation was (25.73 ± 8.65). The patients' stigma score in the dimension of self-depreciation was negatively correlated with their treatment adherence (r=-0.194,P=0.03). Conclusions Nursing staff shall strengthen psychological care for patients and reduce their stigma. Meanwhile, they should take targeted nursing measures from different perspectives and improve patients' treatment adherence.
