儿科重症监护病房患儿血糖水平与病情进展的相关性研究
Association between glucose level and disease severity in children admitted to PICU
目的 探讨儿科重症监护病房(PICU)患儿血糖水平与病情进展的关系.方法 回顾性分析2012年1月至2013年12月本院PICU收治的262例患儿的临床资料,根据患儿入院时血糖水平将其分为正常血糖组112例,轻度升高组78例,重度升高组72例,对比分析3组患儿的血糖水平、危重病例评分(PCIS)、急性生理学及慢性健康评分(APACHE Ⅱ)、简化急性生理评分(SAPS Ⅱ)、全身性感染相关性器官功能衰竭评分(SOFA)及其预后情况.结果 与正常组及轻度升高组相比,重度升高组APACHEⅡ评分、SAPS Ⅱ评分、SOFA评分较高,而PICS评分则较低,差异有统计学意义(P<0.05).重度血糖升高组并发症发生率、病死率显著高于轻度升高组及正常组,差异有统计学意义(P<0.05).结论 通过动态监测PICU患儿血糖水平对预测患者病情进展及预后具有重要的意义.
更多Objective To investigate the association between glucose level and disease severity in children admitted to pediatric intensive care unit (PICU).Methods The clinical data of 262 children admitted to PICU in our hospital from January 2012 to December 2013 were retrospectively analyzed.According to their glucose levels on admission,the children were divided into normoglucemia group (the control group,n=112),mild hyperglucemia group (n=78) and severe hyperglucemia group (n=72).Then,the glucose levels,pediatric critical illness score (PCIS),acute physiology and chronic health evaluation scoring system (APACHE Ⅱ),simplified acute physiology score (SAPS Ⅱ),sepsis-related organ failure assessment (SOFA) and the prognosis of the three groups were comparatively analyzed.Results Compared with the control group and the mild hyperglucemia group,the severe hyprglucemia group showed higher scores of APACHE Ⅱ,SAPS Ⅱ and SOFA,and lower PICS (all P<0.05).The complications rate and mortality in severe hyperglycemia group were higher than those in mild hyperglucemia group and control group (P<0.05).Conclusion Dynamic monitoring of blood glucose level in children admitted to PICU may have important implications for predicting disease progression and prognosis.
