目的 探讨慢性缺血性脑血管病患者髓过氧化物酶(MPO)的表达水平及临床意义.方法 对70例颈动脉粥样硬化(AS)斑块形成的患者进行血浆MPO水平测定,同时对照进行超声造影检查.结果 颈动脉超声造影斑块增强组(增强组)与造影未增强组(非增强组)一般资料差异无统计学意义(均P＞0.05)；增强组梗死率为80.5％ (33/41)；非增强组梗死率为44.8％ (13/29),增强组患者的脑梗死率显著高于非增强组(P＜0.01)；血浆MPO水平,颈动脉超声造影增强组患者显著高于非增强组(P＜0.01),脑梗死组患者显著高于非脑梗死组(P＜0.01).结论 AS斑块不稳定性较高患者MPO表达水平显著增高,提示MPO的高水平表达对于慢性脑血管病的诊断具有十分重要的作用,同时辅以超声造影结果,则可讲行明确诊断.

Objective To investigate the expression and clinical significance of myeloperoxidase (MPO) in patients with chronic ischemic cerebrovascular disease.Methods 70 carotid atherosclerotic plaques patients with plasma MPO level in our hospital were measured and taken ultrasound examination at the same time.Results There was no significant difference for those with contrast-enhanced imaging and without increase in the general information (P ＞0.05).The enhanced infarct rate was 80.5％ (33/41).The non-enhanced infarct was 44.8％ (13/29).The carotid plaque ultrasound contrast enhancement in patients with cerebral infarction group was significantly higher than non-enhanced group(P ＜ 0.01 ).In the plasma levels of MPO detection,contrast-enhanced carotid ultrasound group was significantly higher than non-enhanced group( P ＜ 0.01 ).The cerebral infarction patients was significantly higher than non-cerebral infarction group (P ＜ 0.01 ).Conclusion The atherosclerotic plaque instability in patients with high expression level of MPO was significantly higher,which indicated high level expression of MPO in the diagnosis of chronic cerebrovascular disease had a very important role.Supplemented by the results of ultrasound imaging may had a clear diagnosis.
