摘要目的观察各种肺疾患肺性肥大性骨关节病(HPO)的发生率、治疗后HPO的消失率以及骨显像与X线诊断HPO的价值。方法对703例各种肺疾患患者的骨显像作回顾性分析。显像剂为99Tcm-亚甲基二膦酸盐(MDP)。结果①703例各种肺疾患患者HPO的发生率为7.4%，其中肺癌患者HPO发生率为7.6%。②41例肺癌经手术或化疗后HPO消失31例。③32例甲状腺癌和骨癌肺转移患者骨显像无1例发生HPO，但4例鼻咽癌肺转移患者3例为HPO。结论①骨显像出现HPO图像可提示肺疾患的发生；②鼻咽癌肺转移HPO发生率较高，临床上鼻咽癌患者骨显像出现HPO表现可提示发生肺转移。

abstractsObjective　This retrospective study consists of:(1)703 patients with various pulmonary diseases were followed-up after initial bone scanning . The original hypertrophic pulmonary osteoarthropathy (HPO) incidence and the HPO disappearing after clinical treatment, as well as different diagnostic features between bone scan and X-rays, were compared with each other. (2)36 patients with various metastatic lung cancers were followed-up as to investigate the HPO incidence in each kind of pulmonary diseases and its relationship to the therapeutic efficacy of the pulmonary disease. Methods　The whole-body bone scanning, enhanced by regional plane imaging over the legs, was done for each patient with 99Tcm-MDP. Results　(1) The HPO incidence among the 703 patients with various pulmonary diseases was 7.4%, whereas 7.6% in patients with various lung cancers.(2)HPO disappeared in 31 of total 41 patients with lung cancers after operation or chemotherapy.(3)The HPO incidence in the patients with lung metastases from thyroid cancer or bone cancer was 0%. But there were 3 out of 4 cases of nasopharyngeal cancer were found with HPO, i.e. the rate was high up to 75%. Conclusions　The author generally compared the domestic and foreign reports about HPO incidences on bone scan imaging in patients with various pulmonary diseases. The study indicates, based on its clinical evidences, that: (1) The presence of HPO may predict the lung involvement of the disease process. (2) The HPO incidence in patients with nasopharyngeal cancers reached 75%. Thus if HPO sign is found at the bone scanning on the patients with the diagnosis of nasopharyngeal cancer, it may well indicate lung metastasis involvement.
