髂腰肌拉伸振动训练对脑卒中患者步行能力的影响
Effects of iliopsoas tensile vibration training on the walking ability of stroke survivors
目的 观察髂腰肌拉伸振动训练对脑卒中患者步行能力的影响.方法 选取符合纳入标准的脑卒中偏瘫患者30例,按照随机数字表法将其分为观察组和对照组,每组15例.2组均给予常规康复训练,观察组在此基础上给予髂腰肌拉伸振动训练.2组患者均于治疗前、治疗4周后(治疗后)采用表面肌电图记录患侧髋关节主动屈曲时髂腰肌最大自主等长收缩的均方根值(RMS),评估其肌肉兴奋性,并测量患侧髋关节主动关节活动度(AROM);采用步态分析、Berg平衡量表评估其步行及平衡功能.结果 治疗前、2组患者髂腰肌RMS、AROM、步长及步速、BBS评分比较,差异无统计学意义(P>0.05).治疗后,2组患者上述指标均较组内治疗前有所改善(P<0.05),且观察组髂腰肌RMS[(29.71±4.99) μV]、AROM[(25.15±5.86)°]、步长[(0.57±0.08) m]及步速[(0.51±0.06) m/s]、BBS评分[(45.15±6.42)分]较对照组优异,差异有统计学意义(P<0.05).结论 髂腰肌拉伸振动训练可明显提高脑卒中患侧髂腰肌肌肉兴奋性,改善其平衡能力及步行能力.
更多Objective To observe the effect of iliopsoas tensile vibration training on the walking ability of stroke survivors.Methods Thirty stroke survivors were randomly divided into an observation group and a control group,each of 15.Both groups were given traditional rehabilitation treatment,while the observation group was additionally provided with iliopsoas tensile vibration training.Both groups were evaluated in terms of the root mean square (RMS) of iliopsoas,active range of motion (A-ROM) of the hip joint,the kinematic parameters of gait and Berg balance scale (BBS) scores before and after the 4-week treatment.Results Before the treatment there were no significant differences between the two groups in any of the measurements.After the treatment improvement in all of the measurements was observed in both groups,with the average RMS iliopsoas,A-ROM,step length,step velocity and BBS score of the observation group significantly better than those of the control group.Conclusions Tensile vibration training of the iliopsoas can significantly improve the muscle excitability of the affected iliopsoas in stroke survivors,and improve their balance and walking ability.
