摘要目的:探讨椎间植骨融合内固定术对退行性脊柱侧弯患者Cobb角及脊柱功能的影响。方法:回顾性分析2017年1月至2019年6月苏州大学附属常熟医院收治的退行性脊柱侧弯76例患者的临床资料，依据手术方案将其分为两组，对照组38例行椎管减压术，观察组38例行椎间植骨融合内固定术。比较两组治疗效果、Cobb角、脊柱功能、生活质量及术后并发症情况。结果:观察组治疗总有效率(97.37%，37/38)高于对照组(78.95%，30/38)，差异有统计学意义( P<0.05)。术后3个月，观察组Cobb角、脊柱功能障碍(ODI)评分低于对照组( P<0.05)，生活质量指标评分高于对照组( P<0.05)。观察组术后并发症发生率为5.26%(2/38)，低于对照组的23.68%(9/38)，差异有统计学意义( P<0.05)。 结论:椎间植骨融合内固定术治疗退行性脊柱侧弯能减小Cobb角度，改善脊柱功能及患者生活质量。

abstractsObjective:To investigate the effect of interbody fusion and internal fixation on Cobb angle and spinal function in patients with degenerative scoliosis.Methods:The clinical data of 76 patients with degenerative scoliosis admitted to Changshu Hospital Affiliated to Suzhou University from January 2017 to June 2019 were analyzed retrospectively. They were divided into two groups according to the surgical plan, the 38 cases of control group was treated with decompression of spinal canal, the other 38 cases of the observation group underwent interbody fusion and internal fixation. The treatment effect, Cobb angle, spinal function, quality of life and postoperative complications were compared between the two groups.Results:The total effective rate of the observation group (97.37%, 37/38) was higher than that of the control group (78.95%, 30/38), the difference was significant ( P<0.05). Three months after the operation, the Cobb angle and oswestry disability index (ODI)scores of the observation group were lower than those of the control group ( P<0.05), and the scores of quality of life indicators were higher than those of the control group ( P<0.05). The postoperative complication rate in the observation group was 5.26% (2/38), which was lower than 23.68% (9/38) in the control group, and the difference was significant ( P<0.05). Conclusions:The treatment of degenerative scoliosis with interbody fusion and internal fixation can reduce the Cobb angle, improve spinal function and the quality of life of patients.
