摘要目的 探讨银丹心脑通软胶囊联合酒石酸美托洛尔对老年冠心病不稳定性心绞痛(UAP)患者心电图及血清脑钠肽(BNP)、可溶性细胞间黏附分子(sICAM-1)水平变化的影响.方法 选取2015年1月至2017年2月本院收治的102例老年UAP患者,随机数字表法分组,各51例.对照组采用常规抗心绞痛治疗,在此基础上,研究组采用银丹心脑通软胶囊+酒石酸美托洛尔,两组持续治疗3个月.观察对比两组心电图疗效、心功能指标[左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)、左心室收缩末期内径(LVESD)]变化情况、血清BNP、sICAM-1水平变化情况、不良反应发生率.结果 经秩和检验,研究组心电图总有效率为94.12％(3/51),高于对照组的74.51％ (38/51),差异有统计学意义(P＜0.05);治疗后研究组LVESD、LVEDD低于对照组,LVEF高于对照组,差异均有统计学意义(均P＜ 0.05);治疗后研究组血清BNP、sICAM-1水平均低于对照组,差异均有统计学意义(均P＜0.05);研究组不良反应发生率为17.64％(9/51),与对照组的21.56％(11/51)相比,差异无统计学意义(P＞0.05).结论 银丹心脑通软胶囊联合酒石酸美托洛尔可提高老年UAP患者心电图疗效,降低血清BNP、sICAM-1水平,改善心功能,且安全性较高.

abstractsObjective To investigate the effects of Yindanxinnaotong soft capsule combined with metoprolol tartrate on electrocardiogram and serum levels of brain natriuretic peptide (BNP),and soluble intercellular adhesion molecule (sICAM-1) in elderly patients with coronary heart disease and unstable angina pectoris (UAP).Methods From January 2015 to February 2017,102 elderly patients with UAP treated at our hospital were randomly divided into a control group and a study group,51 cases for each group.The control group were treated with conventional anti-angina pectoris.In addition,the study group were treated with Yindanxinnaotong soft capsule and metoprolol tartrate.Both groups were treated for 3 months.The ECG efficacy,heart function indicators [Left ventricular end diastolic diameter (LVEDD),left ventricular ejection fraction (LVEF),left ventricular end systolic diameter (LVESD)],serum levels of BNP and sICAM-1,and the incidence of adverse reactions were observed and compared between the two groups.Results The total effective rate of electrocardiogram was higher in the study group than in the control group [94.12％ (3/51) vs.74.51％ (38/51)],with a statistical difference (P ＜ 0.05).After treatment,the LVESD and LVEDD and the serum levels of BNP and sICAM-1 were lower and the LVEF was higher in the study group than in the control group,with statistical differences (all P ＜ 0.05).The incidence of adverse reactions was 17.64％ (9/51) in the study group and 21.56％ (11/51) in the control group,with a statistical difference (P ＞ 0.05).Conclusions Yindanxinnaotong soft capsule combined with metoprolol tartrate for elderly patients with UAP can improve the efficacy of ECG,decrease the serum levels of BNP and sICAM-1,and improve heart function and is safe.
