目的了解脊柱骨折合并脊髓损伤患者出院准备度水平,探讨其影响因素.方法? 采用方便抽样法,选取2016年12月—2017年12月在温州医科大学附属第二医院脊柱外科收治的即将出院的脊柱骨折合并脊髓损伤患者237例为研究对象,采用一般资料调查问卷、出院准备度量表以及社会支持评定量表对其进行调查,并采用单因素及多元线性回归分析出院准备度的影响因素.结果? 患者出院准备度总分为(79.81±11.48)分,维度得分由高到低依次为适应能力(35.97±6.80)分、预期性支持(25.77±4.98)分、个人状态(18.07±4.30)分.多元线性回归分析显示,社会支持、家庭人均月收入、年龄、出院后方便就医、文化程度是脊柱骨折合并脊髓损伤患者出院准备度的主要影响因素(偏回归系数分别为0.834、3.249、-1.973、-6.523、3.487;P＜0.05).结论? 脊柱外科合并脊髓损伤患者的出院准备度不足,医护工作者应着重关注高龄、受教育程度低、经济状况不佳、就医不便以及社会支持低的患者的出院准 备度,加强患者院外延续性管理,进而提高患者出院准备度,促进院外康复.

Objective? To understand the readiness for hospital discharge of spine fracture patients combined with spinal injury and to explore its influencing factors. Methods? From December 2016 to December 2017, we selected 237 spine fracture patients combined with spinal injury about to leave hospital in Department of Orthopedics at the Second Affiliated Hospital of Wenzhou Medical University as subjects by convenience sampling. All of the patients were investigated with the general information questionnaire, readiness for hospital discharge scale and social support rating scale. Single factor analysis and multiple linear regression analysis were used to explore the influencing factors of the readiness for hospital discharge. Results? Among those patients, the total score of the readiness for hospital discharge was (79.81±11.48), and dimension scores ranged from high to low including coping ability (35.97±6.80), expected support (25.77±4.98) and personal status (18.07±4.30). Multiple linear regression analysis showed that the main influencing factors of readiness for hospital discharge of spine fracture patients combined with spinal injury included social support, average income per person in family, ages, being convenient to seek medical advice and education levels (β=0.834, 3.249, -1.973, -6.523, 3.487;P＜0.05). Conclusions? Spine fracture patients combined with spinal injury are with the insufficient readiness for hospital discharge. Medical staff should pay attention to readiness for hospital discharge among patients with advanced ages, low level of education, poor economic conditions, being inconvenient to seek medical advice and low level of social support, and strengthen continuous management of patients outside the hospital so as to improve patients＇ readiness for hospital discharge and promote rehabilitation outside the hospital.
