目的 观察术前外周血中性粒细胞与淋巴细胞比值(NLR)和血小板与淋巴细胞比值(PLR)对较低级别胶质瘤患者临床预后的影响.方法 共纳入经术后病理证实的初发较低级别胶质瘤病例242例,收集其临床资料及生存资料.采用Kaplan-Meier法计算生存率,用Log-rank进行单因素生存分析.Cox模型比例风险评估做多因素分析.结果 NLR和PLR的最佳cut-off值分别为1.95和197.22.NLR≤1.95和NLR＞ 1.95两组的3年和5年生存率分别为0.809比0.649、0.675比0.462,两者差异有统计学意义(P＜0.01);PLR≤197.22和PLR＞ 197.22两组3年和5年生存率分别为0.756比0.549、0.599比0.275,两者差异有统计学意义(P＜0.01),高的NLR和PLR与较低级别胶质瘤患者差的临床预后明显相关.NLR(P ＜0.01)、PLR(P＜0.05)是影响较低级别胶质瘤患者临床预后的独立危险因素.结论 高的NLR和PLR预示较低级别胶质瘤患者较差的临床预后.

Objective To investigate the prognostic values of neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) in patients with lower-grade gliomas.Methods 242 patients with pathological diagnosis of lower-grade gliomas (WHO Ⅱ and WHO Ⅲ) were enrolled in the study,and relevant data were collected.Kaplan-Meier method was used to calculate the survival rates.Cox regression model was used to conduct multivariate analysis.Results The best cut-off values for NLR and PLR were 1.95 and 197.22.The 3-year and 5-year survival rate for NLR ≤ 1.95 and NLR ＞ 1.95 subgroups were 0.809 vs.0.649 and 0.675 vs.0.462,respectively (P ＜ 0.01).The 3-year and 5-year survival rate for PLR≤ 197.22 and PLR ＞ 197.22 subgroups were 0.809 vs.0.649 and 0.675 vs.0.462,respectively (P ＜0.01).High NLR (P ＜0.01) and PLR (P ＜0.05) were independent prognostic factors influencing the survival of lower-grade gliomas.Conclusion NLR and PLR were potential factors for predicting the survival of lower-grade gliomas.
