摘要口服胶原蛋白肽有促进胶原蛋白代谢、预防和治疗骨关节炎、骨质疏松等健康效应。循证营养学是利用现有的、系统评述的最佳证据来进行营养政策的制定和营养行动的实施。目的:为了评估目前胶原蛋白肽研究的证据级别和推荐强度。方法:我们使用关键词胶原蛋白肽、胶原蛋白水解物、水解胶原蛋白、骨和关节检索PubMed资料库2014年以前的文献,根据循证营养学的五步循证法:提出问题、检索证据、评价证据的级别和推荐强度、应用证据和自我评估,来分析胶原蛋白肽的骨与关节健康效应研究结果的证据级别和推荐强度。结果:(1)服用胶原蛋白肽Peptan治疗膝骨关节炎的随机双盲、安慰剂对照研究(B级);(2) Effect of collagen hydrolysate in articular pain:A 6-month randomized, double-blind, placebo controlled study(B级); (3)Change in knee osteoarthritis cartilage detected by delayed gadolinium enhanced magnetic resonance imaging following treatment with collagen hydrolysate:a pilot randomized controlled trial (B级); (4) 24-Week study on the use of collagen hydrolysate as a dietary supplement in athletes with activity-related joint pain (B级); (5) Effects of Pro-Hyp,a Collagen Hydrolysate-Derived Peptide, on Hyaluronic Acid Synthesis Using in Vitro Cultured Synovium Cells and Oral Ingestion of Collagen Hydrolysates in a Guinea Pig Model of Osteoarthritis (体外实验和动物试验); (6) Stimulation of type Ⅱ collagen biosynthesis and secretion in bovine chondrocytes cultured with degraded collagen(体 外实验); (7) Chondroprotective effect of the bioactive peptide prolyl-hydroxyproline in mouse articular cartilage in vitro and in vivo(动物试验和体外研究);(8)Collagen hydrolysate for the treatment ofosteoarthritis and other joint disorders:a review of the literature (B级); (9) Biomarkers, type Ⅱ collagen, glucosamine and chondroitin sulfate in osteoarthritis follow-up: the "Magenta osteoarthritis study"(B级);(10)A randomized controlled trial on the efficacy and safety of a food ingredient, collagen hydrolysate, for improving joint comfort (B级)。 (1 1)胶原素对老龄小 鼠骨强度和成分的影响(动物试验); (12) Hydrolyzed collagen improves bone metabolism and biomechanical parameters in ovariectomized mice: an in vitro and in vivo study(动物试验和体外研究);(13) Hydrolyzed collagen improves bone status and prevents bone loss in ovariectomized C3H/HeN mice(动物试验); (14)口服PeptanTM 胶原蛋白肽对中老年膝骨关节炎妇女骨代谢生化指标和骨密度的影响(D级,临床研究)。结论:循证营养学 分析显示:上海第六人民医院的"服用胶原蛋白肽Peptan治疗膝骨关节炎的随机双盲、安慰剂对照研究"为国内首创,国际领先水平。属于循证医学证据B级,是单个研究级别最高的,具有较高的推荐强度。
