摘要目的:探讨延续性护理在心脏机械瓣置换患者华法林抗凝治疗中的应用效果。方法:对行心脏机械瓣置换手术需终生口服华法林抗凝治疗的100例患者，随机分为对照组与研究组各50例。对照组给予常规出院指导，研究组在对照组的基础上实施华法林抗凝治疗的延续性护理，分别在出院后第1个月、3个月、6个月、12个月对患者抗凝治疗相关依从性知识、国际标准化比值（INR）检测及并发症的发生率进行测评。结果:通过延续性护理，研究组抗凝治疗相关依从性知识得分显著高于对照组，研究组INR检测达标比例显著高于对照组，出血及栓塞并发症的发生率显著低于对照组，差异均有统计学意义（均 P<0.05）。 结论:延续性护理能有效提高患者院外自我护理能力，提高华法林抗凝治疗效果，降低患者并发症的发生率。

abstractsObjective:To investigate the application effect of continuous nursing in anticoagulation therapy with warfarin in patients with mechanical valve replacement.Methods:A total of 100 patients with mechanical valve replacement who needed oral administration of warfarin all their life for the anticoagulation therapy were selected as study objects, and were divided into the research group ( n=50) and the control group ( n=50) randomly. The control group only received discharge guidance, and the research group received the continuous nursing of anticoagulation therapy with warfarin on the basis of the control group. The patients were assessed for compliance with anticoagulant therapy, international normalized ratio (INR), and the incidence of complications at 1 month, 3 months, 6 months, and 12 months after discharge. Results:Through continuous nursing, the compliance knowledge score of anticoagulant therapy in the research group was significantly higher than that in the control group, the rate of INR test reaching the standard in the research group was significantly higher than that in the control group, the incidence of complications (bleeding and embolization) was significantly lower than that in the control group (all P<0.05). Conclusion:Continuous nursing can effectively improve the self-care ability of patients outside the hospital, improve the anticoagulant therapy effect of warfarin, and reduce the incidence of complications.
