胸腔镜辅助肋骨骨折内固定+血胸清除术治疗多发肋骨骨折的研究
Thoracoscopic assisted internal fixation of rib fractures + hemothorax removal for multiple rib fractures
摘要目的:观察评价分析在基层医院应用胸腔镜辅助多发肋骨骨折内固定+血胸清除术治疗多发肋骨骨折(Multiple rib fractures,MRF)的临床效果。方法:选择2017年2月至2019年2月期间本院收治多发肋骨骨折30例,采用胸腔镜辅助多发肋骨骨折内固定+血胸清除术治疗,并观察治疗后的临床效果。结果:30例患者电视胸腔镜手术均获成功,术后并发症为术后持续胸壁疼痛,无手术死亡;术中出血量(47.28±2.91)ml,手术时间(83.94±6.36)min (68~141 min),术后胸腔引流管放置时间(5.16±1.48)d (4~12 d),术后引流液总量(173.47±20.83)ml (48~396 ml),疼痛视觉模拟(VAS)评分为(3.48±1.16)分,住院时间(7.05±1.32)d(4~13 d)。结论:胸腔镜辅助多发肋骨骨折内固定+血胸清除术治疗肋骨多发骨折在基层医院实施术式简便、易于操作,是安全、有效、理想的治疗方法,适合在基层医院大力开展应用。

abstractsObjective:To observe and evaluate the clinical effect of the application of thoracoscopic assisted internal fixation of rib fractures + hemothorax removal for multiple rib fractures (MRF) in primary hospitals.Methods:Thirty patients with multiple rib fractures were treated in our hospital from February 2017 to February 2019, who were treated with thoracoscopic assisted internal fixation of multiple rib fractures + hemothorax removal, and the clinical effects after treatment were observed.Results:The video-assisted thoracoscopy was successful in all 30 patients. The postoperative complications were persistent chest wall pain without surgical death. The average blood loss during the operation was (47.28±2.91) ml, the operation time was 68-141 min, with the average of (83.94±6.36) min. The postoperative thoracic drainage tube was placed for 4 to 12 days, with the average of (5.16±1.48) days. The total amount of postoperative drainage fluid was 48 to 396 ml, with the average of (173.47±20.83) ml, the VAS score was (3.48±1.16) points, and the hospital stay was 4 to 13 days, with the average of (7.05 ± 1.32) days.Conclusion:Thoracoscopic assisted internal fixation of multiple rib fractures + hemothorax removal for multiple rib fractures is simple and easy to operate in primary hospitals. It is a safe, effective, and ideal treatment method, and it is suitable for vigorous application in primary hospitals.
