Onyx胶稀释技术在脑动静脉畸形介入治疗中的应用
Application of Onyx dilution technology in interventional treatment for cerebral arteriovenous malformation
摘要目的:探讨Onyx胶稀释技术在脑动静脉畸形介入栓塞治疗中的应用价值。方法:选取本院2020年9月至2021年3月收治的脑动静脉畸形患者40例作为研究对象,其中男25例,女15例,年龄(32.58±12.69)岁;根据Spetzler-Martin分级分为两个研究小组(Ⅰ~Ⅲ级24例为一组,Ⅳ~Ⅴ级16例为另一组),两组患者以随机等量方式分别进入对照组(单纯Onyx-18栓塞)和试验组(Onyx-18联合Onyx稀释技术栓塞);比较对照组和试验组的栓塞效果和预后情况。结果:对照组和试验组完全栓塞率分别为10.00%(2/20)和25.00%(5/20),差异无统计学意义( P>0.05);试验组栓塞体积比为(73.25±20.08)%,明显高于对照组的(40.50±29.69)%,差异有统计学意义( P<0.05);对照组有2例患者出现并发症,试验组1例,两组比较差异无统计学意义( P>0.05);对照组和试验组术后1周的GOS评分分别为(3.55±1.28)分和(3.65±1.04)分,两组比较差异无统计学意义( P>0.05)。 结论:结合Onyx胶稀释技术,能明显改善脑动静脉畸形的介入栓塞效果,可作为常规手段的有效补充,研究结论需要进一步大样本多中心研究进行验证。

abstractsObjective:To explore the value applying Onyx dilution technology in interventional embolization for cerebral arteriovenous malformation.Methods:Forty (32.58±12.69) years old patients with cerebral arteriovenous malformation treated at our hospital from September, 2020 to March, 2021, including 25 males and 15 females, were selected as the research objects, and divided into two research groups based on the Spetzler-Martin grading: 24 cases of grade I-Ⅲ were included into a group, and another 16 cases of grade Ⅳ-V were included into another group. The patients in the two groups were divided into a control group, treated by Onyx-18 embolization, and an experimental group, treated by Onyx-18 and Onyx dilution technology embolization, in a random equivalent manner. The embolization effects and prognosis conditions were compared between the control group and the experimental group.Results:The complete embolism rate was 10.00% (2/20) in the control group, and was 25.00% (5/20) in the experimental group, with no statistical difference ( P>0.05). The embolization volume ratio was obviously higher in the experimental group than in the control group [(73.25±20.08)% vs. (40.50±29.69)%; P<0.05]. Two patients had complications in the control group, and one in the experimental group, with no statistical difference ( P>0.05). One week after the operation, the GOS score was (3.55±1.28) in the control group, and was (3.65±1.04) in the experimental group, with a statistical difference ( P>0.05). Conclusions:It can obviously improve the interventional embolization effect of cerebral arteriovenous malformation by combining with the Onyx dilution technology. It can be regarded as an effective complement for the conventional methods, and the research conclusion shall be further verified with a large sample and by multicenter studies.
