摘要患者女,31岁.因左上唇及左额部外伤瘢痕4年余,局部斑块3个月就诊.皮肤科检查:左上唇、额部皮肤分别见2cm×3.5 cm和1.5 cm×1.5 cm的斑块,色淡红,边界清楚,质硬.皮损组织病理检查:真皮内可见大小不一的细胞团块,由上皮样细胞及少许多核巨细胞构成,无明显干酪样坏死.PAS、PAM 及抗酸染色阴性,各系统实验室检查未见异常.诊断:瘢痕性结节病.予激光磨削术,口服甲泼尼龙、匹多莫德和阿奇霉素综合治疗,取得良好疗效.随访1年,治疗区局部平整,且恢复正常肤色.

abstractsA 31-year-old female patient was admitted to the hospital for traumatic scars in the left upper lip and forehead for more than 4 years and localized plaques for 3 months.Skin examination revealed 2 slightly erythematous,well-marginated,indurated plaques sized 2 cm × 3.5 cm and 1.5 cm × 1.5cm respectively in the left upper lip and forehead.Histopathology of the lesions revealed varying sizes of cell clumps composed of epithelioid cells and a few multinucleated giant cells in the dermis,while no obvious caseous necrosis was found.In addition,periodic acid-Schiff (PAS)staining,periodic acid methenamine silver (PAM) staining and acid-fast staining were negative.Laboratory tests revealed no abnormality in other organ systems.The patient was diagnosed with scar sarcoidosis,and treated with laser resurfacing as well as oral methylprednisolone,pidotimod and azithromycin,which resulted in a satisfactory outcome.After one-year follow-up,the lesion was flattened with a return to normal skin colour.
