摘要目的 探讨腮腺区腺样囊性癌对面神经的侵袭与疗效的关系.方法 对56例患者病历资料进行回顾性研究.结果 腺样囊性癌在腮腺区的不同部位及病史的长短与面神经是否侵袭及治疗效果密切相关,切缘全阴性的患者3年生存率100%,复发率12%,5年生存率100%,复发率26%,切缘存在阳性的患者3年生存率100%,复发率34%,5年生存率85%,复发率46%.结论早发现、早治疗可有效的避免神经被侵袭;肿瘤扩大切除及面神经追踪切除,切缘阴性可显著的提高生存质量,降低复发率.

abstractsObjective To investigate the relation between the invasion and efficacy of adenoid cystic carcinoma of parotid gland to facial nerve. Methods Clinical data of 56 cases were retrospectively analyzed. Results The Different parts and the history of adenoid cystic carcinoma in the parotid area was closely related to the facial nerve whether the invasion and the treatment effect or not. the 3-year survival rate of Whole margin-negative patients was 100% ,the relapse rate was 12% ,the 5-year survival rate was 100% ,the relapse rate was 26% ,the 3-year survival rate of margin-pesitive patients was 100% ,relapse rate was 34% ,5-year survival rate was 85 %, the recurrence rate was 46%. Conclusion The early detection and early treatment is effective to a-void the invasion of nerve; extended resection of the tumor, phased resection tracking of facial nerve and negative resection margin can improve the quality of life and reduce the relapse rate significantlly.
