CTA模拟手术入路在颅内动脉瘤夹闭术中的应用
Application of Simulating Operative Approach by CTA in Intracranial Aneurysm Clipping
摘要目的 探讨CT血管造影(CTA)模拟手术入路在颅内动脉瘤夹闭术中的价值.方法 16例颅内破裂动脉瘤患者术前在仅行CTA,未行DSA.利用CTA模拟手术入路,后处理采用容积成像(VR)技术,采集图像与术中所见相比较.结果 16例动脉瘤病例仅在CTA图像资料的基础上均顺利完成动脉瘤夹闭术.CTA图像很好的模拟手术人路.依据CTA所模拟的手术人路影像均可顺利找到动脉瘤.CTA对颅内动脉瘤解剖结构、载流动脉、周围解剖标志等重要结构显示清晰,与术中所见高度一致,对术中辨认解剖结构有很大帮助.结论 CTA能清晰显示颅内动脉瘤及其周围解剖结构,与术中所见高度一致.术前利用CTA模拟手术入路,可以为动脉瘤夹闭手术提供重要信息

abstractsObjective To investigate the value of simulating operative approach by CT angiography (CTA) in intracranial aneurysm clipping.Methods 16 patients with intracranial aneurysm underwent CTA without DSA before clipping.Operative approach was simulated by CTA.Iinages were processed with volume randering (VR) and compared with surgical findings.Results 16 patients were all succeed in aneurysm clipping on the bases of CTA without DSA.Operative approach was well simulated by CTA.All aneurysms were found successfully with the help of CTA images.The anatomy of aneurysm,parent arteries and anatomic marks were accordant with sursical findings.Conclusion CTA can display intracranial aneurysm clearly.The anatomy of aneurysm and its surroundings were accordant with surgical findings.To simulate operative approach by CTA can provide important information for aneurysm clipping before surgery.
