摘要目的 探讨七氟醚吸入复合利宁凝胶涂布气管镜表面在小儿气管异物取出术麻醉的效果.方法 选择136例气管异物患儿,年龄10个月～5岁.随机分为3组:A组(56例)患儿用γ-羟丁酸钠、氯胺酮复合麻醉;B组(40例)用七氟醚吸入麻醉;C组(40例)麻醉方法与B组相同,只是在气管镜表面涂抹利宁凝胶.三组患儿分别在麻醉诱导前(T0)、诱导后(T)、置镜1 min(T1)、2 min(T2)、5 min(T3)观察HP、SpO2变化,以及声门开放、术中呛咳、屏气程度、苏醒时间、术者对麻醉满意度.结果 A、B两组患儿T2、T3时刻心率上升,SpO2下降,与T时刻比较P＜0.05,C组T2、T3时刻心率、SpO2无明显变化,与T时刻比较P＞0.05;C组术中并发症低于A、B两组(P＜0.05);手术医生对麻醉满意度以C组最高.结论 七氟醚吸入复合利宁凝胶涂布气管镜表面用于小儿气管异物取出术的麻醉具有HP、SpO2平稳,术中并发症少、苏醒快、术者满意度高等优点.

abstractsObjective To explore the anaesthetic effect of inhalational anesthesia of sevoflurane and lining germ used for intratracheal topical anesthesia for tracheal foreign body removal in children. Methods The chosen 136 tracheal foreign body children aged from 10 mouthes to 5 years old,were randomly divided into 3 groups: the group A (56 children ) are anesthetized with Sodium, γ-Hydroxybutrate and ketamine; the group B (40 children)used inhalational anesthesia of sevoflurane; the group C (40 children)was the same as the group B,besides Lining germ used for intraltracheal topical anesthesia. We observd many symptoms of the three groups, such as HR, SpO2 and glottis opening breathholding, bucking, crying, and disgorging etc, before induction (T0), after steady anesthesia induction (T), at the moment of putting scope 1 min ( T1 )、 2 min ( T2 )、 5 min(T3). Results The group A and B children's RR increased at T2 and T3 moments, SpO2 decreased (P ＜0.05), Compared with T moment, the group C children's RR and SpO2 had no obvious changes at T2 and T3 moments, compared with T moment(P ＞ 0.05 ). The group C's complication was lower than that of group B and A, ( P ＜ 0.05). The surgical doctors are satisfied with the group C. Conclusion There is so much good quality such as smooth HP、SpO2 ,low complication and quick waking up etc for tracheal foreign body removal in children with inhalational anesthesia of sevoflurane and lining germ used for intraltracheal topical anesthesia.
