摘要目的:探讨对急性髓系白血病(AML)诱导化学治疗并发口腔黏膜炎(OM)患者采用重组人粒细胞巨噬细胞集落刺激因子(rhGM-CSF)辅助治疗的效果及对创面愈合时间的影响。方法:选取2017年1月至2019年1月庆阳市中医院血液病科收治的48例AML诱导化学治疗并发OM患者，男25例，女23例，年龄(39.58±5.18)岁，年龄范围为22～55岁。采用随机数表法将所有患者随机分为复方氯己定组和联合rhGM-CSF组，每组24例。复方氯己定组患者予以复方氯己定含漱液治疗，联合rhGM-CSF组在复方氯己定组的基础上与rhGM-CSF含漱液交替治疗。治疗30 d后，观察两组患者临床治疗有效率、OM疼痛缓解程度、OM愈合时间及治疗期间不良反应的发生情况。结果:联合rhGM-CSF组患者OM治疗有效率[87.5%(21/24)]高于复方氯己定组[62.5%(15/24)]、OM疼痛缓解率[87.5%(21/24)]高于复方氯己定组[58.3%(14/24)]、OM愈合时间[(21.42±3.26)d]短于复方氯己定组[(23.59±4.16)d]、治疗期间不良反应发生率[12.5%(3/24)]低于复方氯己定组[45.8%(11/24)]，差异均有统计学意义( P<0.05)。 结论:对AML诱导化学治疗并发OM患者采用rhGM-CSf含漱液联合复方氯己定含漱液交替治疗可提高患者治疗有效率，有效缓解疼痛，加快OM创面愈合，减少不良反应发生。值得在临床上推广应用。

abstractsObjective:To investigate the effect of Recombinant human granulocyte macrophage colony stimulating factor(rhGM-CSF)adjuvant therapy on acute myeloid leukemia(AML)induced chemotherapy complicated with OM and its effect on wound healing time.Methods:A retrospective study was performed on 48 cases of patients with AML induced chemotherapy for OM patients who were admitted from January 2017 to January 2019, 25 males and 23 females, aged(39.5±5.18)years old, ranging from 22 to 55 years old.All patients were randomly divided into the compound chlorhexidine group and rhGM-CSF group, with 24 cases in each group.Patients in the compound chlorhexidine group were treated with compound chlorhexidine gargle, and patients in combination with rhgm-csf group were treated alternately with rhGM-CSF gargle on the basis of compound chlorhexidine group.After 30 days of treatment, the effective rate of clinical treatment, degree of OM pain relief, OM healing time and occurrence of adverse reactions during treatment in the two groups were observed.Results:Joint rhGM-CSF OM total effective treatment group patients[87.5%(21/24)]above compound chlorhexidine group[62.5%(15/24)], OM pain relief rate[87.5%(21/24)]above compound chlorhexidine group[58.3%(14/24)], OM healing time short[(21.42 ± 3.26)days]from that of the group compound chlorhexidine[(23.59±4.16)days], the incidence of adverse reactions during the treatment[12.5%(3/24)]below compound chlorhexidine group[45.8%(11/24)], and the differences were statistically significant( P<0.05). Conclusion:For patients with OM complicated with AML induction chemotherapy, RhGM-CSf gargle combined with compound chlorhexidine gargle can improve the effective rate of treatment, effectively relieve pain, speed up OM wound healing, and reduce adverse reactions.It is worthy of clinical application.
