摘要目的 探讨B超检查对开放性眼外伤Ⅰ期术后伤眼检查的临床应用价值.方法 对2006年11月至2008年3月收治的46例经玻璃体切割手术治疗的开放性眼外伤,Ⅰ期术后病人进行回顾性研究,分析每眼术前B超声像图特征,并结合玻切术中眼内情况,分析两者的一致性.结果 开放性眼外伤Ⅰ期术后B超表现复杂多样,84.78%伤眼合并两种以上并发症,结合术中所见,各项并发症B超表现符合率分别为:玻璃体混浊100%,晶体脱位95.65%,玻璃体机化膜90.48%,球内或球壁异物66.67%,视网膜脱离58.62%,睫状体或脉络膜脱离45.45%.结论 开放性眼外伤并发症多而复杂,Ⅰ期清创缝合术后往往因屈光间质混浊无法观察眼内组织结构情况,应用B超检查可以帮助了解晶状体、玻璃体和视网膜的受损情况,为Ⅱ期手术方法及时机,特别是玻璃体视网膜手术提供可靠依据.

abstractsObjective To evaluate the application and value of B-scan after primary operation of complicated open-ocular trauma.Method 46 cases of post-primary operation of complicated open-ocular trau-ma with pars plana vitrectomy from November 2006 to March 2008 were analysed retropectively.Their B-scan ultrasound image characteristics were compared seperetely with the results during the vitectomy.Results The performance of B-scan after primary operation of complicated open-ocular trauma is complex and diverse. There are more than two complications in 87.78% eyes.The corresponding rate of Vitreous opacity,Lens dislo-cation,Vitreous membrane,Intraocular foreign and Retinal detachment diagnosed by B-scan are 100%, 95.65% ,90.48% ,66.67% ,58.62% and 45.45%.Conclusion The complications of open-ocular trauma are complex and diverse.It is difficult to observe vitreous and retinal because of opacity of ocular media.B-scan is a powerful tool for ocular injury diagnosis, especially before the pars plana vitrectomy.
