摘要胰腺癌恶性程度高，预后差，手术是唯一可治愈的方法。近年来，微创手术在胰腺外科中发展迅速，但胰腺癌常伴有血管侵犯和炎症粘连，增加了微创手术的难度，其可行性、安全性、肿瘤根治性等方面一直存在争议。本文结合现有指南与最新临床研究进展，比较微创与开放手术在胰腺癌中的应用，总结胰腺癌微创治疗的发展现状与争议。外科医生应严格把握微创手术适应证，进一步开展胰腺癌微创治疗临床研究，从而规范并提升我国微创胰腺外科的发展。

abstractsPancreatic cancer is extremely malignant and has poor prognosis. Surgery is the only way for the therapy. In recent years, minimally invasive surgery has been developed rapidly in pancreatic surgery. However, pancreatic cancer is often accompanied with vascular aggression and inflammation, which greatly increasing the difficulty of minimally invasive surgery procedure. Its feasibility, safety and radical tumor cure are still controversial. In this article, combined with the existing guidelines and the latest clinical researches, we compared the application of minimally invasive and open surgery in pancreatic cancer and summarized the development status and controversy of minimally invasive treatment of pancreatic cancer. Surgeons should strictly grasp the minimally invasive surgical adaptation certificate, and further carry out clinical research on minimally invasive treatment of pancreatic cancer, so as to standardize and enhance the development of minimally invasive pancreatic surgery in China.
