摘要目的 通过分析各种影响特发性面神经炎疗效的因素,评价高压氧(HBO)综合治疗的效果.方法 回顾性分析196例特发性面神经炎患者的治疗,采用统计学方法分析患者性别、年龄、基础疾病、HBO治疗、H-B分级和面神经传导速度等可能影响疗效的因素.根据患者是否行HBO治疗将其分为常规治疗组(102例)和HBO综合治疗组(94例).结果HBO治疗、H-B分级和面神经传导速度是影响疗效的主要因素;HBO综合治疗对H-B分级为Ⅱ一Ⅵ级患者的疗效均高于常规治疗组,尤其对Ⅲ-Ⅳ级和Ⅴ-Ⅵ级的患者的疗效与常规治疗组相比差异有统计学意义(P<0.01);对面神经传导速度轻、中和重度减慢的患者,HBO综合治疗组所有患者的疗效高于常规治疗组,其中,对传导速度呈中、重度减慢的患者疗效与常规治疗组比较差异有统计学意义(P<0.01).结论HBO对不同程度的特发性面神经炎患者均有良好疗效,尤其在中、重度患者中疗效显著.早期HBO综合治疗有利于提高疗效.

abstractsObjective To analyze different factors influencing on therapeutic effect of idiopathic facial neuritis and to evaluate effect of hyperbaric oxygen (HBO) combined therapy on them. Methods The influencing factors were investigated statistically through a retrospective analysis on total therapeutic effect of196 patients with idiopathic facial palsy, including sex, age, basic disease, HBO treatment, house-brackmann facial grading and facial nerve conduction velocity. According to the different methods of treatment, all patients were divided into routine treatment group (102 cases) and HBO treatment group (94 cases). Therapeutic effects of the two groups were compared. Results HBO treatment, house-brackmann facial grading and facial nerve conduction velocity were major influencing factors. The total effective rate of group B was higher than that of group A, particularly in patients whose house-brackmann facial grading was Ⅲ-Ⅳ and Ⅴ- Ⅳ grade(P<0.01)and whose facial nerve conduction velocity stepped down in middling and serious degrees(P<0.01).Conclusions The curative effect of HBO on patients with idiopathic facial neuritis of different degree is significant, particularly in patients with middling and serious degree. It will help to improve therapeutic effect to receive HBO treatment early.
