高压氧综合治疗对人工膝关节置换术后患者的镇痛效果观察

摘要目的 探讨高压氧(HBO)治疗用于人工膝关节置换术后患者的镇痛效果.方法 选取庆阳市人民医院骨一科择期行膝关节置换术患者80例为研究对象,按照随机数字表法分为观察组和对照组,每组40例.2组患者均给予传统镇痛治疗,观察组在传统镇痛基础上加用HBO治疗.分别于术后2、6、24和48 h、5和7 d以视觉模拟评分(VAS)法评估2组患者静息痛和运动痛,并记录2组患者术后补救镇痛次数.结果 观察组术后6、24和48 h、5和7 d静息痛VAS评分均低于同时间点对照组,差异有统计学意义(P<0.05或P<0.01).观察组术后48 h、5和7 d静息痛VAS评分均低于同时间点对照组,差异有统计学意义(P<0.05或P<0.01).观察组与对照组术后给予补救镇痛次数分别为2次、10次,观察组补救镇痛率(5.0%)明显低于对照组(25.0%),差异有统计学意义(P<0.01).结论 HBO综合治疗人工膝关节置换术后对患者镇痛有一定的效果,该镇痛模式具有一定的临床应用前景.

abstractsObjective To investigate the analgesic effect of hyperbaric oxygen ( HBO) on the patients after artificial knee arthroplasty. Methods Eighty patients who underwent elective knee replacement surgery in the Orthopedics Department of the Qingyang Peaple' s Hospital were selected as study subjects, and were equally divided into the control and the observation groups. The patients of the 2 groups were all given traditional analgesic treatment, and the patients of the observation group were additionally treated with HBO on the basis of traditional analgesic treatment. At 2, 6, 24, 48 and hours, as well as at 5 and 7 days after surgery, visual analogue scale ( VAS) were used to record the pain at rest and with movement and the number of postoperative remedial analgesia in the patients of the 2 groups. Results At 2, 6, 24 and 48 hours, as well as at 5 and 7 days after surgery, the VAS pain scores at rest for the observation group were all significantly lower than those of the control group at the same time points, however, no statistical significance could be seen, when comparisons were made in the VASs at rest 2 hours after surgery between the 2 groups(P>0. 05). At 6, 24 and 48 hours, as well as at 5 and 7 days after surgery, statistical significance could all be noticed, when comparisons were made in the VAS scores at rest between the 2 groups(P <0. 05). The VAS scores with movement after surgery for the observation group at different time points were all lower than those of the control group at the same time points. There was no evident statistical significance in the VAS scores with movement at 2, 6 and 24 hours time points after surgery, when comparisons were made between the patients of the 2 groups (P>0. 05). Statistical results indicated that the VAS scores with movement at 48 hour, and 5 and 7 days after surgery for the observation group were obviously superior to those of the control group(P<0. 05). The number of postoperative remedial analgesia for the observation group and the control group were respectively twice and 10 times, and statistical significance could be observed in the number of postoperative remedial analgesia when&nbsp;comparisons were made between the 2 groups(P < 0. 05). Conclusion HBO therapy in artificial knee arthroplasty could produce certain therapeutic effects and is worth further close study.
