摘要目的:探讨单唾液酸四己糖神经节苷脂钠(MGS)联合高压氧(HBO)治疗外伤癫痫的临床疗效。方法:选取2017年1月至2019年9月烟台山医院神经内科收治的外伤癫痫患者86例作为研究对象。按照数字表法随机分为观察组和对照组，每组43例；对照组给予常规抗癫痫药物及MGS进行治疗，观察组在对照组的治疗基础上给予HBO治疗。比较2组患者治疗的有效率、脑电图转归情况、癫痫每个月发作频率和发作持续时间及临床症状改善情况。结果:治疗3个疗程后，2组患者发作频率和发作持续时间均明显低于治疗前，且观察组患者治疗后癫痫发作频率和发作持续时间显著低于和短于对照组，差异均有统计学意义( P<0.05)。观察组患者单纯部分发作、复杂部分发作、全面性发作类型癫痫的治疗总有效率均明显高于对照组，差异均有统计学意义( P<0.05)。观察组患者治疗后的总有效率(95.3%)明显高于对照组总有效率(74.4%)，差异有统计学意义( χ2＝4.481， P<0.05)。 结论:MGS联合HBO治疗外伤癫痫，可明显减少神经元的异常放电，降低癫痫的发作频率，减少发作后的持续时间，可临床推广应用。

abstractsObjective:To investigate the clinical effect of monosialotetrahexosylganglioside sodium (MGS) combined with hyperbaric oxygen (HBO) in the treatment of post-traumatic epilepsy.Methods:A total of 86 patients with posttraumatic epilepsy, treated at the Neurology Department of Yantai Mountain Hospital, from January 2017 to September 2019, were selected as study objects and randomly divided into the observation group and the control group by using the random number table, with 43 cases in each group. The control group was treated with conventional antiepileptic drugs and MGS, and the observation group was treated with HBO on the basis of the treatment in the control group. The effective rate, electroencephalography (EEG) outcome, seizure onset frequency per month, seizure duration, and improvement of clinical symptoms were compared between the two groups.Results:After three courses of treatment, the onset frequency and duration of seizure in the two groups were significantly lower than those before treatment, and the onset frequency and duration of seizure in the observation group were significantly lower than those in the control group ( P<0.05). The total effective rates of the treatment for the patients with simple partial seizure, complex partial seizure, and generalized seizure in the observation group were significantly higher than those in the control group, and the differences were statistically significant ( P<0.05). After treatment, the total effective rate of the observation group (95.3%) was significantly higher than that of the control group (74.4%), and the difference was statistically significant ( χ2=4.481, P<0.05). Conclusion:MGS combined with HBO in the treatment of post-traumatic epilepsy can significantly reduce the abnormal discharge of neurons, the seizure onset frequency, and the seizure duration; which is worthy of popularization in clinical practice.
