摘要本文分析64名40~88岁中、老年健康人75g葡萄糖耐量、胰岛素和C肽释放曲线。ΣΔIRI与ΣΔC-PR有明显相关(P<0.05)。FBG、FIRI、FC-PR和BG、IRI、C-PR三种曲线面积在男女间均无显著差异(P>0.05)。比较上述6项指标,肥胖组(≥标准体重120%,18名)各项均值都高于非肥胖组(≤106%,18名),只有FBG与ΣΔIRI之差别有统计学意义。将46名非肥胖者分为40~49、50~59、60~88岁三组进行分析。发现FBG,ΣΔBG、FC-PR、ΣΔIRI、ΣΔC-PR、ΣΔIRI/ΣΔBG,与ΣΔC-PR/ΣΔBG各项均值均随年龄递增。经方差分析FBG、FC-PR、ΣΔIRI及ΣΔC-PR有显著意义。以上结果提示老年人对葡萄糖负荷的胰岛素分泌反应增强。又上述之比值在60~88岁组明显高于40~49岁组(P<0.05),提示老年人β细胞功能没有衰退,反而有亢进倾向。

abstractsSeventy five gm glucose tolerance test together with insulin and C-peptide release curve were performed in 64 healthy volunteers, aged from 40 to 88 years. The ΣΔIRI and ΣΔCPR values were significantly correlated (P<0.05). Comparison between the obese and the non-obese groups revealed that the obese group gave higher values of such parameters as FBG, FIRI, FCPR, ΣΔBG, ΣΔIRI and ΣΔCPR but the differences were significant only for FIRI and ΣΔIRI. By subdividing the 46 non-obese subjects into three different age groups the relationship of glucose metabolism and secretory capacity of ?-cells of pancreatic islets with aging was analyzed with the following results: The mean values of FBG, ΣΔBG, FCPR, ΣΔIRI, and ΣΔCPR as well as the ratios of ΣΔIRI/ΣΔBG and ΣΔCPR×100/ΣΔBG apparently increased with aging. These changes were statistically significant for FBG, FCPR, ΣΔIRI and ΣΔCPR when analyses of variance were used. The conclusion is that, along with decreasing glucose tolerance that occurred with aging, the response of insulin secretion when challenged with glucose was enhanced. Furthermore, the ratios of ΣΔIRI/ΣΔBG and ΣΔCPR×100/ΣΔBG likewise were higher in the 60-88 year than in the 40-year groups (P<0.05). It is speculated that the ?-cells of elderly subjects might be hyperfunctioning rather than hypofunctioning as expected.
