摘要目的 探讨社区康复中心糖尿病综合管理效果. 方法 选取2015年1月至2016年12月我社区康复中心管理区域内220例2型糖尿病患者,按照随机数字表法将其分为常规组(110例)和综合组(110例),常规组患者采取普通防治管理,综合组患者采取社区康复中心综合防治管理,并比较不同组患者管理效果. 结果 管理后综合组患者空腹血糖和糖化血红蛋白水平均低于常规组和管理前(均P＜0.05);管理后,综合组患者对药物治疗、运动锻炼、尿糖或血糖监测、饮食依从性、自我管理能力等评分均高于常规组(均P＜0.05);管理后综合组患者低血糖事件(11.9％和21.3％,x2 =5.088,P=0.024)、末梢神经病变(13.8％和28.1％,x2＝9.986,P＝0.002)和视网膜病变(12.5％和26.3％,x2 =9.682,P＝0.002)等并发症发生率低于常规组. 结论 糖尿病综合防治管理有助于保持糖尿病患者血糖稳定,提高患者自我管理水平以及减少并发症发生.

abstractsObjective To explore the effectiveness of a management approach involving community rehabilitation based comprehensive prevention and treatment of diabetes.Methods From January 2015 to December 2016,220 patients with type 2 diabetes in our community were randomly divided into a conventional group (n =110)and a comprehensive group (n-110).The conventional group took usual prevention and treatment,and the comprehensive group received community-based comprehensive prevention and management.The two groups were compared on their effectiveness.Results After intervention,fasting blood glucose and glycosylated hemoglobin levels in the comprehensive group were significantly lower than those in the conventional group and pre-intervention levels in the same group(each P ＜ 0.05);Scores on drug therapy,exercise,urine glucose or blood glucose monitoring,dietary compliance,and self-management ability were significantly higher in the comprehensive group than in the conventional group (all P ＜ 0.05);The incidences of hypoglycemia(11.9％ and 21.3％,x2 =5.088,P=0.024),peripheral neuropathy(13.8％ and 28.1％,x2 =9.986,P =0.002),and retinopathy(12.5％ and 26.3％,x2 =9.682,P =0.002)were significantly lower than those in the conventional group.Conclusions Community rehabilitation center-based comprehensive management not only maintains blood sugar stability for diabetic patients,but also improves patients' self-management ability and reduces the occurrence of complications.
