摘要多发性骨髓瘤(multiple myeloma，MM)是一种浆细胞异常增殖的血液系统恶性肿瘤，好发于老年人。老年MM患者早期治疗死亡风险大、药物不良事件增多，生存时间较年轻患者明显缩短。对老年患者进行衰弱评估，同时对其合并症及并发症进行全程管理有利于判断预后并合理选择用药。本共识采用改良德尔菲法，通过对31名血液病学专家完成两轮问卷调查，分别就老年MM患者衰弱评分系统选择及用药调整，对心血管疾病、周围神经病变、感染、糖尿病、血栓、骨质疏松及第二肿瘤等合并症的管理，以及药物依从性、心理、营养状况、康复和镇痛等方面进行相关推荐，以期对临床治疗具有一定指导意义。

abstractsMultiple Myeloma(MM)is a malignant hematological disease characterized by abnormal proliferation of plasma cells, predominantly occurring in the old population.The early treatment of elder MM presents a high risk of death rate and an increase in adverse drug events, with a significant shorter survival when compared to younger patients.Conducting frailty assessments on elderly patients and managing their complications throughout the treatment process can bring an aid in prognosis determination and in rational drug choices.This consensus was developed using a modified Delphi method, through two rounds of questionnaire surveys completed by 31 hematological experts.It offers recommendations on the selection of frailty scoring systems and medication adjustments, management of complications such as cardiovascular diseases, peripheral neuropathy, infections, diabetes, thrombosis, osteoporosis, and secondary tumors, as well as on medication adherence, psychological, nutritional status, rehabilitation, and pain management, with the aim of providing guidance for clinical treatment.
