衰弱评估及其对老年胃癌手术临床结局影响的研究进展
Research progress on impact of frailty on clinical outcomes of gastric cancer surgery in elderly patients and its assessment methods
摘要衰弱是一种以生理储备下降和脆弱性增加为特点的非特异性状态,被广泛应用于评估老年患者术前状态。胃癌是全球常见的恶性肿瘤,老年患者日益增加,手术是可切除胃癌的首选治疗方式。老年患者因其功能下降,术后不良事件发生率高,准确评估患者术前状态有望提高其手术预后。文章就衰弱评估及对老年胃癌手术临床结局的影响展开综述,阐述了目前常见的几种衰弱评估工具及不同国家和地区选择的倾向性,总结了衰弱对术后病死率、并发症、生存质量及医疗花费的影响,发现衰弱可显著增加术后短期和长期死亡率、使Clavien-Dindo(CD)分级>2患者的并发症发生率显著增高,并降低术后生存质量,但其对癌症特异性生存期及医疗花费的影响暂不明确。

abstractsFrailty is a syndrome characterized by decreased physiological reserves and increased vulnerability, which is widely used to evaluate the preoperative status of elderly patients. Gastric cancer is a common malignant tumor worldwide, with an increasing number of elderly patients, and surgery is the preferred treatment for resectable gastric cancer. Due to the decline in function and high incidence of postoperative adverse events in elderly patients, accurate assessment of their preoperative status is expected to improve their surgical prognosis. Frailty can significantly increase short-term and long-term postoperative mortality, significantly increase the incidence of complications with Clavien-Dindo (CD) grade>2, and reduce postoperative quality of life; however, its impact on cancer specific survival and medical expenses is unclear currently. This article reviews the impact of frailty on the clinical outcomes of elderly patients after gastric cancer surgery, including postoperative mortality, complications, quality of life and medical expenses; it also summarizes the commonly used frailty assessment tools and the selection preference in different countries and regions.
