摘要目的:探讨胃结直肠癌患者血脂水平[包括总胆固醇(CHOL)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、甘油三脂(TG)]与外周血循环肿瘤细胞(CTCs)数量的关系。方法:分析2020年1月至2022年12月在华中科技大学附属协和医院收治的92例胃结直肠癌症患者的临床资料，应用CTCBIOPSY系统分离并检测CTCs，应用 t检验、非参数检验及Spearman相关性检验分析患者血脂指标如CHOL、LDL、HDL、TG与CTCs相关性。 结果:在46例胃癌患者中，CTCs低表达28例(60.9%)，高表达18例(39.1%)，其中CHOL和HDL在CTCs各表达组差异有统计学意义( t＝－2.278、－2.806， P<0.05)，LDL和TG在CTCs各表达组差异无统计学意义( t＝－1.753、 Z＝－1.712， P>0.05)；在46例结直肠癌患者中，CTCs低表达25例(54.3%)，高表达21例(45.7%)，CHOL、LDL、HDL和TG在不同表达水平CTCs差异无统计学意义( t＝－0.238、 Z＝－0.099、 t＝1.341、－0.235， P>0.05)；经Spearman分析可得出胃结直肠患者的CHOL，HDL和LDL水平与CTCs数量密切相关( rs＝0.235、0.30、0.2223， P<0.05)，TG水平则无相关( rs＝－0.059， P>0.05)。 结论:部分胃结直肠癌患者血脂指标水平异常，且与CTCs数量明显相关，说明高循环血脂水平可能影响癌细胞的侵袭及转移水平，与肿瘤的发生发展有关。早期对肿瘤患者进行降脂干预也许会辅助肿瘤治疗，改善患者预后，延长生存时间。

abstractsObjective:To investigate the correlation between blood lipid levels [including total cholesterol (CHOL), low-density lipoprotein (LDL), high-density lipoprotein (HDL), triglyceride (TG)] and the number of circulating tumor cells (CTCs) in the peripheral blood in patients with gastric and colorectal cancer.Methods:The clinical data of 92 patients with colorectal cancer admitted to the Union Hospital of Huazhong University of Science and Technology from January 2020 to December 2022 were retrospectively analyzed. CTCs were separated and detected using the CTCBIOPSY system, and the correlation between blood lipid indicators such as CHOL, LDL, HDL, TG, and CTCs was analyzed by the t-test, nonparmetric test and Spearman test. Results:Among 46 gastric cancer patients, 28 cases (60.9%) showed low expression of CTCs and 18 cases (39.1%) showed high expression. Among them, there was significant difference in CHOL and HDL expression among the CTCs expression groups ( t=-2.278, -2.806, P<0.05). There was no significant difference in LDL and TG expression among the CTCs expression groups ( t=-1.753, Z=-1.712, P>0.05). Among 46 colorectal cancer patients, 25 cases (54.3%) had low expression of CTCs and 21 cases (45.7%) had high expression of CTCs. There was no significant difference in CTCs at different expression levels of CHOL, LDL, HDL, and TG ( t=-0.238, Z=-0.099, t=1.341, -0.235, P>0.05). The Spearman analysis showed that the levels of CHOL, HDL and LDL in gastric and colorectal patients were closely related to the number of CTCs ( rs=0.235, 0.330, 0.223, P<0.05), but the levels of TG was not ( rs=-0.059, P>0.05). Conclusion:Some patients with gastric and colorectal cancer have abnormal levels of blood lipid indicators, which are correlated with the number of CTCs in the peripheral blood. This suggests that high circulating blood lipid levels may affect the invasion and metastasis levels of cancer cells, and are related to the occurrence and development of cancer. Early lipid-lowering interventions for cancer patients may assist in cancer treatment, improve prognosis of patients, and extend survival time.
