窄带成像在儿童过敏性结肠炎内镜检查中指导靶向活检的价值
The value of narrow-band imaging for targeting biopsy of lesions in children with allergic colitis
摘要目的:探讨窄带成像(NBI)在儿童过敏性结肠炎内镜检查中指导靶向活检的价值。方法分别在传统白光内镜和 NBI 模式下对结直肠进行观察、记录,在2种模式下分别进行活检,以内镜下活检的组织病理学作为最终诊断,比较普通白光内镜活检结果、NBI 靶向活检结果与病理诊断结果的关系。结果共完成60例,NBI 下病变检出直肠病变为47例(78.3%),乙状结肠52例(86.7%),降结肠为19例(31.7%),横结肠、升结肠、回盲部较少,分别为21例(35.0%)、25例(41.7%)、8例(13.3%)。对于横结肠、升结肠部病变 NBI 下的发现率均明显高于白光内镜[35.0%比16.7%(10/60),P<0.05;41.7%比18.3%(11/60),P<0.05],乙状结肠、直肠病变发现率 NBI 与白光内镜比较差异均无统计学意义[86.7%比83.3%(50/60),P>0.05;78.3%比76.7%(46/60),P>0.05],回盲部、降结肠病变发现率 NBI 与白光内镜比较差异亦均无统计学意义[13.3%比5.0%(3/60),P>0.05;31.7%比28.3%(17/60),P>0.05]。活检结果 NBI 下的靶向活检病理提示固有层嗜酸性粒细胞(EOS)≥6个/ HP 比例明显高于白光下活检结果,差异有统计学意义( P<0.05)。结论 NBI技术操作简便,可清晰观察病灶微细结构,有助于提高早期过敏性结肠炎的靶向活检准确率,在常规肠镜检查中发现可疑病灶后,可采用 NBI 观察,指导靶向活检。

Methods The colorectal lesions under the model of traditional white light imaging and NBI in children with allergic colitis were observed and recorded. The targeted biopsy results by NBI indicated that the ratio of EOS≥6/ HP was significantly higher than the biopsy results by white light imaging. Conclusion NBI is easy to operate and can discern the microcosmic structure which cannot be observed by the traditional endoscopy. It can provide higher diagnostic accuracy of allergic colitis. After suspicious lesions were found under conventional endoscopic check,NBI could be used to guide the targeted biopsy.
