摘要目的:分析单光子发射计算机断层及X射线计算机体层摄影成像系统(SPECT/CT)肺灌注显像对肺栓塞的诊断价值。方法:纳入2019年3月至2022年3月开封市中心医院收治的80例疑似肺栓塞患者为研究对象，进行前瞻性研究，对所有患者实施SPECT/CT肺灌注显像检查，以肺动脉造影检查结果为金标准，分析SPECT/CT肺灌注显像对肺栓塞的诊断效能及对肺栓塞类型的评估价值。结果:80例疑似肺栓塞患者中肺栓塞71例(88.75%)，非肺栓塞9例(11.25%)。以肺动脉造影检查结果为金标准，SPECT/CT肺灌注显像诊断肺栓塞的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为90.14%(64/71)、88.89%(8/9)、90.00%(72/80)、98.46%(64/65)、53.33%(8/15)；SPECT/CT肺灌注显像评估肺栓塞类型与肺动脉造影检查结果的一致性极好( Kappa＝0.852， P<0.05)。 结论:SPECT/CT肺灌注显像对肺栓塞具有良好诊断价值，可准确评估肺栓塞类型。

abstractsObjective:To analyze the diagnostic value of single photon emission computed tomography and X-ray computed tomography (SPECT/CT) pulmonary perfusion imaging for pulmonary embolism.Methods:A total of 80 patients with suspected pulmonary embolism treated in Kaifeng Central Hospital from March 2019 to March 2022 were enrolled the study as research objects. SPECT/CT pulmonary perfusion imaging was performed on all patients. And the results of pulmonary angiography were taken as the gold standard, to analyze diagnostic efficacy of SPECT/CT pulmonary perfusion imaging for pulmonary embolism and its value in evaluating the type of pulmonary embolism.Results:Among the 80 patients with suspected pulmonary embolism, 71 cases (88.75%) had pulmonary embolism and 9 cases (11.25%) had non pulmonary embolism. With results of pulmonary angiography as the gold standard, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of SPECT/CT pulmonary perfusion imaging in diagnosing pulmonary embolism were 90.14% (64/71), 88.89% (8/9), 90.00% (72/80), 98.46% (64/65) and 53.33% (8/15), respectively. The consistency between SPECT/CT pulmonary perfusion imaging and pulmonary angiography was excellent ( Kappa=0.852, P<0.05) in evaluation of pulmonary embolism type. Conclusions:SPECT/CT pulmonary perfusion imaging has good diagnostic value for pulmonary embolism, and it can accurately evaluate the type of pulmonary embolism.
