TSH、TPOAb、TRAb及TG-Ab联合检测诊断自身免疫性甲状腺疾病的价值
Value of combined detection of TSH, TPOAb, TRAb and TG-Ab in the diagnosis of autoimmune thyroid disease
摘要目的:分析促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(TPOAb)、促甲状腺激素受体抗体(TRAb)及甲状腺球蛋白抗体(TG-Ab)联合检测诊断自身免疫性甲状腺疾病的价值。方法:抽取2021年1月至2022年6月聊城市第三人民医院收治的自身免疫性甲状腺疾病患者160例设为甲组,将同期收治的非自身免疫性甲状腺疾病患者52例设为乙组,另抽取参与体检的39例正常人设为丙组。检测并比较三组实验室指标(TSH、TPOAb、TRAb及TG-Ab)水平,统计4项联合检测及单项检测的诊断效能。结果:甲组TSH、TPOAb、TRAb、TG-Ab水平高于乙组及丙组,且乙组高于丙组,差异有统计学意义( P<0.05)。4项联合检测诊断自身免疫性甲状腺疾病的敏感度(98.13%,157/160)及准确率(97.21%,244/251)高于TSH单测(89.38%,143/160;90.84%,228/251)、TPOAb单测(91.25%,146/160;91.63%,230/251)、TRAb单测(84.38%,135/160;88.05%,221/251)、TG-Ab单测(81.25%,130/160;83.27%,209/251),特异性(95.60%,87/91)高于TG-Ab单测(86.81%,79/91), P<0.05。 结论:TSH、TPOAb、TRAb及TG-Ab联合检测诊断自身免疫性甲状腺疾病准确率较高,诊断价值高。

abstractsObjective:To analyze the value of combined detection of thyroid stimulating hormone (TSH), thyroid peroxidase antibody (TPOAb), thyroid stimulating hormone receptor antibody (TRAb), and thyroglobulin antibody (TG-Ab) in the diagnosis of autoimmune thyroid disease.Methods:A total of 160 patients with autoimmune thyroid disease admitted to Liaocheng Third People’s Hospital from January 2021 to June 2022 were selected as group A, and 52 patients with non-autoimmune thyroid disease during the same period were selected as group B, and 39 healthy people who participated in physical examination were selected as group C. The levels of laboratory indicators (TSH, TPOAb, TRAb and TG-Ab) of the three groups were detected and compared. The diagnostic efficacy of combined detection and single detection of the 4 indicators were counted.Results:The levels of TSH, TPOAb, TRAb, and TG-Ab in group A were higher than those in group B and group C, and the above indicators in group B were higher than those in group C, with significant difference ( P<0.05). The sensitivity (98.13%, 157/160) and accuracy (97.21%, 244/251) of the combined detection of the 4 indicators in diagnosing autoimmune thyroid disease were higher than those of TSH single detection (89.38%, 90.84%, 228/251), TPOAb single detection (91.25%, 146/160; 91.63%, 230/251), TRAb single detection (84.38%, 135/160; 88.05%, 221/251), TG-Ab single detection (81.25%, 130/160; 83.27%, 209/251), moreover, specificity (95.60%, 87/91) of combined detection was higher than that of TG-Ab single detection (86.81%, 79/91), P<0.05. Conclusions:The combined detection of TSH, TPOAb, TRAb and TG-Ab has a high accuracy in diagnosing autoimmune thyroid disease, and its diagnostic value is significant.
