摘要目的:探讨复方丹参注射液联合苯巴比妥治疗新生儿缺氧缺血性脑病(HIE)的效果，并分析其对神经行为、血清神经递质水平的影响。方法:回顾性抽取2020年3月至2023年3月西安交通大学第一附属医院榆林医院收治的HIE患儿90例，依据治疗方案将其分为对照组(45例，予以苯巴比妥治疗)和观察组(45例，予以复方丹参注射液联合苯巴比妥治疗)。比较两组临床疗效、临床症状改善时间、新生儿行为神经测定(NBNA)评分、脑电图、脑血流量、血清神经递质[5-羟色胺(5-HT)、多巴胺(DA)、去甲肾上腺素(NE)]水平。比较两组不良反应及后遗症发生率。结果:观察组总有效率高于对照组，临床症状改善时间短于对照组( P<0.05)。治疗后，观察组NBNA评分、脑血流量高于对照组，脑电图棘波波幅低于对照组( P<0.05)。治疗后，观察组血清5-HT、DA、NE水平高于对照组，观察组后遗症发生率低于对照组( P<0.05)。两组不良反应发生率比较差异未见统计学意义( P>0.05)。 结论:复方丹参注射液联合苯巴比妥治疗新生儿HIE疗效显著，可加快机体康复进程，促进神经行为恢复，增加脑血流量，调节神经递质分泌，降低后遗症发生率。

abstractsObjective:To investigate the therapeutic effect of the combination of compound Danshen injection and phenobarbital on neonates with hypoxic-ischemic encephalopathy (HIE), and to analyze its influence on neurobehavior and level of serum neurotransmitter.Methods:A total of 90 children with HIE admitted to Yulin Hospital, the First Affiliated Hospital of Xi’an Jiaotong University from March 2020 to March 2023 were retrospectively selected, and they were divided into control group (45 cases, treated with phenobarbital) and observation group (45 cases, treated with compound Danshen injection combined with phenobarbital) according to the treatment plan. The clinical efficacy, time of clinical symptoms remission, neonatal behavioral neurological assessment (NBNA) score, electroencephalogram, cerebral blood flow, and serum neurotransmitters including 5-hydroxytryptamine (5-HT), dopamine (DA) and norepinephrine (NE) were compared between the two groups. The incidences of adverse reactions and sequelae of the two groups were compared.Results:The total effective rate of the observation group was higher than that of the control group, and the time of clinical symptoms remission in the observation group was shorter than that in the control group ( P<0.05). After treatment, the NBNA score and cerebral blood flow of the observation group were higher than those of the control group, and the amplitude of the electroencephalogram spike wave in the observation group was lower than that in the control group ( P<0.05). After treatment, the levels of serum 5-HT, DA and NE in the observation group were higher than those in the control group, and the incidence of sequelae in the observation group was lower than that in the control group ( P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P>0.05). Conclusions:Compound Danshen injection combined with phenobarbital has significant efficacy in the treatment of neonatal HIE, which can accelerate the process of physical rehabilitation, promote neurobehavioral recovery, increase cerebral blood flow, regulate neurotransmitter secretion, and reduce the incidence of sequelae.
