With the increase in number of minimally invasive thoracic proceduresintraoperative one-lung ventilation is being used more often than before. Double-lumen tubes are the most commonly used devices for lung isolation. A fibreoptic bronchoscope is essential to aid placementconfirm position and troubleshoot problems that arise with use of lung isolation devices. A clear plan is essential in managing patients with difficult airway (including tracheostomy) who require one-lung ventilation. Hypoxaemia with initiation of one-lung ventilation is common and should be managed in a stepwise sequential manner.
