摘要目的 调查和分析目前本院急性阑尾炎围手术期抗菌药物的使用情况,为急性阑尾炎围手术期抗菌药物的合理使用提供依据.方法 回顾性分析2009年1月至2010年12月本院急性阑尾炎围手术期抗菌药物使用的病历资料,依据《抗菌药物临床应用指导原则》,判断是否合理使用抗菌药物.结果 本院急性阑尾炎围手术期抗菌药物的使用存在较明显的不合理现象,主要表现为:术后用药种类和方式不合理(46.2％)、术前预防性用药种类不合理(32.5％)和术后使用抗菌药物时间过长(25.6％).不合理使用抗菌药物增加了术后感染的发生( 34.2％ vs11.1％,P=0.014)、延长了患者的住院时间(8.1±3.6) dvs (5.5± 2.4)d,P=0.036.结论 目前急性阑尾炎围手术期不合理使用抗菌药物仍较为常见,需药学部门加强监督和指导.

abstractsObjective To provide support for logical antibiotic uses by investigating irrational use of antibiotics during the perioperative period in acute appendicitis in our hospital.Methods The data of antibiotic uses during the perioperative period in acute appendicitis from January 2009 to December 2010 were retrospectively analyzed.The uses of antibiotics were assessed according to Guidelines for Clinical Uses of Antibiotics.Results Irrational use of antibiotics during the perioperative period in acute appendicitis obviously existed in our hospital,including improper types and ways of postoperative antibiotic uses ( 46.2％ ),improper types of preoperative uses( 32.5％ ),and prolonged antibiotic uses after surgery ( 25.6％ ).Infection rates and length of hospital stay increased owing to irrational use of antibiotics.Conclusions Irrational use of antibiotics during the perioperative period in acute appendicitis is still common.Supervision and instructions for antibiotic therapy should be strengthened.
