摘要目的 对比研究全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折的临床疗效.方法 42例老年股骨颈骨折患者按不同手术治疗方式分为全髋组(27例,行全髋关节置换术治疗)和半髋组(15例,行半髋关节置换术治疗),对两组治疗效果进行综合比较.结果 半髋组手术时间和术中出血量明显少于全髋组(P＜0.05);两组术后髋部疼痛、假体松动、下肢深静脉血栓、关节脱位的发生率比较差异无统计学意义(P＞0.05);全髋组远期优良率(96.30％)明显高于半髋组(66.67％)(P＜0.05).结论 全髋关节置换术治疗老年股骨颈骨折远期疗效显著,半髋关节置换术具有手术时间短、术中出血少等优势,临床应根据患者具体情况选择最佳治疗方案.

abstractsObjective To compare the clinical efficacy between total hip replacement (THA) and half hip replacement (HHA) in treating gerontal femoral neck fracture.Methods 42 cases of gerontal femoral neck fracture patients were divided into THA group (27 cases,given THA) and HHA group (15 cases,given HHA),the efficacy of two groups were compared.Results The operative time,blood loss of HHA group were less than those of THA group (P ＜ 0.05); there was no statistical significant difference in the incidence of hip pain,implant loosening,deep venous thrombosis and joint dislocation between two groups (P ＞ 0.05);the forward excellent-rate of THA group (96.30％) was higher than that of HHA group (66.67％) (P ＜ 0.05).Conclusion There is a significant forward effect on THA in treating gerontal femoral neck fracture,HHA has many advantages such as shorter operative time,less blood loss,so the best and reasonable treatment approach should be selected according to patients' condition.
