摘要目的 观察内分泌调控对经鼻蝶窦内镜切除混合型垂体瘤后患者术后内分泌、生活质量、病情复发率等的影响.方法 选取本院2010年1月至2016年12月期间神经外科收治的82例混合型垂体瘤患者,按随机数字法分为观察组(41例)和对照组(41例),两组患者均行经鼻蝶窦内镜切除混合型垂体瘤手术,对照组患者予以术后生活习惯调控措施,观察组在生活习惯调控基础上予以生理剂量激素治疗,对照组术后不予任何内分泌调控治疗.比较两组术后1个月血清内分泌激素水平[生长激素(GH)、泌乳素(PRL)、甲状腺激素(T3、T4)、促甲状腺激素(TSH)、血皮质醇(F)及促肾上腺皮质激素(ACTH)]、慢性鼻-鼻窦炎临床评价量表(RQLQ)、生活质量SF-36量表评分及术后1年复发率.结果 观察组治疗1个月后血清(PRL、GH、TSH、ACTH、F)表达水平、SF-36量表评分、RQLQ量表评分显著低于对照组1个月后(P＜0.05),观察组1个月后血清T3、T4表达水平显著高于对照组1个月后(P＜0.05).观察组病情复发率明显低于对照组(4.88％比21.95％,P＜0.05).结论 术后内分泌调控治疗可改善经鼻蝶窦内镜切除混合型垂体瘤患者的内分泌激素水平,提升患者术后生存质量,降低术后病情复发率.

abstractsObjective To observe the effects of endocrine regulation on endocrine,quality of life,and recurrence of patients with mixed pituitary adenoma after transsphenoidal endoscopy.Methods In department of neurosurgery of our hospital from January 2010 to December 2016,82 patients with mixed pituitary tumors were randomly divided into observation group (41 cases) and control group (41 cases).All patients underwent transsphenoidal endoscopic resection for mixed pituitary adenoma.The control group received postoperative lifestyle control measures,and the observation group received physiological dose hormone therapy on the basis of lifestyle regulation.Serum endocrine hormone levels (PRL,GH,T3,T4,TSH,F,ACTH),RQLQ scale score,quality of life (SF-36 scale) score 1 month after surgery,and 1-year postoperative recurrence rate were compared between the two groups.Results 1 month after surgery,serum endocrine hormone levels (PRL,GH,TSH,F,ACTH),RQLQ scale score,SF-36 scale score in the observation group were significantly lower than those in the control group (P＜0.05),serum levels ofT3 and T4 in the observation group were significantly higher than those in the control group (P＜0.05).The recurrence rate of the observation group (4.88％) was significantly lower than that of the control group (21.95％) (P＜0.05).Conclusion Postoperative endocrine regulation can improve endocrine hormone levels in patients with mixed pituitary tumors undergoing transsphenoidal endoscopic resection,improve postoperative quality of life,and reduce postoperative recurrence rate.
