摘要目的 探讨幽门螺杆菌(HP)感染与IgA肾病自然病程的关系.方法 收集本院2015年1月至2018年12月收治的IgA肾病患者236例,依据HP检查结果分为阳性组104例和阴性组132例.对两组患者尿液、血液常用肾病评估指标;并采用Katafuchi评分对肾脏穿刺活检结果进行比较.结果 两组尿白蛋白、24h尿蛋白定量、尿微量白蛋白、隐血、IgA1及血白蛋白、总胆固醇、甘油三酯、高密度脂蛋白胆固醇-C及低密度脂蛋白胆固醇-C差异均有统计学意义(P＜0.05).阳性组Katafuchi各项评分及总分均高于对照组,差异有统计学意义(P＜0.05).结论 HP感染在IgA肾病自然进程中发挥促进作用,对于合并HP感染的IgA肾病患者,抗HP治疗有可能为缓解IgA肾病进展提供新思路.

abstractsObjective To investigate the relationship between Helicobacter pylori (HP) infection and the natural course of IgA nephropathy.Methods 236 patients with IgA nephropathy admitted to our hospital from January 2015 to December 2018 were divided into positive group (104 cases) and negative group (132 cases) according to the results of HP examination.The nephropathy evaluation indexes of urine and blood were compared between the two groups,and the results of renal biopsy were compared by Katafuchi score.Results There were statistically significant differences in urinary albumin,24-hour urinary protein,urinary microalbumin,occult blood,IgA1,and serum albumin,total cholesterol,triglyceride,high density lipoprotein cholesterol-C,and low density lipoprotein cholesterol-C between the two groups (P＜0.05).The scores of each item of Katafuchi score and total score of the positive group were higher than those of the negative group (P＜0.05).Conclusion HP infection plays an important role in the natural process of IgA nephropathy.Anti-HP therapy may provide new ideas for alleviating the progression of IgA nephropathy in patients with HP infection.
