摘要目的 探讨甘草人参汤联合奥美拉唑对上消化道出血(UGIH)患者的临床效果,并分析其对患者血清中TNF-α、IL-2水平的影响.方法 收集2016年3月至2018年8月本院收治的UGIH患者80例,按照数表法随机分成两组,即对照组(40例,采用奥美拉唑治疗)和观察组(40例,采用甘草人参汤+奥美拉唑治疗).两组均连续治疗四周后,对比两组患者治疗有效率、治疗前后指标变化、不良反应发生情况以及TNF-α、IL-2水平的影响.结果 观察组患者的总有效率(92.50％,37/40)显著高于对照组(80％,32/40),差异有统计学意义(P＜0.05).治疗后观察组出现恶心、呕吐、呕血、头晕、便血以及黑便例数显著低于对照组(P＜0.05).治疗后观察组血清中TNF-α[(105.06±13.17)ng/ml比(296.84±16.57)ng/ml]、IL-2水平[(10.23±6.10)ng/ml比(24.54±8.73)ng/ml]以及TNF-α/IL-2比值[(8.55±4.52)比(12.81±3.16)]均显著低于对照组(P＜0.05).结论 UGIH患者给予甘草人参汤联合奥美拉唑治疗,能有效提高治疗有效率,纠正机体IL-2、TNF-α细胞因子水平,可为临床上UGIH的治疗提供依据.

abstractsObjective To investigate the clinical effect of liquorice ginseng decoction combined with omeprazole in patients with upper gastrointestinal hemorrhage(UGIH),and to analyze the effect on serum TNF-α and IL-2 levels.Methods A total of 80 UGIH patients admitted to our hospital from March 2016 to August 2018 were randomly divided into two groups according to the number table method,namely control group(40 cases treated with omeprazole)and observation group(40 cases treated with liquorice ginseng decoction and omeprazole).After four weeks of continuous treatment,the two groups were compared in terms of treatment efficiency,changes before and after treatment,adverse reactions,and the effects on TNF-α and IL-2 levels.Results The total effective rate of the observation group(92.50％,37/40)was significantly higher than that of the control group(80％,32/40),with statistically significant difference(P＜0.05).After treatment,the incidence of adverse reactions such as nausea,vomiting,hematemesis,dizziness,blood in the stool,and melena in the observation group were significantly lower than those in the control group(P＜0.05).After treatment,TNF-α level,IL-2 level,and TNF-α/IL-2 ratio in the observation group were significantly lower than those in the control group[(105.06±13.17)ng/ml vs.(296.84±16.57)ng/ml,(10.23±6.10)ng/ml vs.(24.54±8.73)ng/ml,(8.55±4.52)vs.(12.81±3.16)](P＜0.05).Conclusion Liquorice ginseng decoction combined with omeprazole can effectively improve the treatment efficiency of UGIH patients.correct the levels of IL-2 and TNF-α cytokines in the body,which Call provide a basis for the treatment of UGIH in clinical treatment.
