以居住地域为基础的广州地区结核病传播链分子流行病学初探
Preliminary explore on molecular epidemiology of tuberculosis transmission chains in Guangzhou based on residence
摘要目的:探讨MIRU-VNTR基因分型技术用于分析广州地区结核病传播关系的可行性。方法:以结核患者居住地所属乡镇村组或社区街道相同为前提,假定居住所属地相同结核患者间存在传播关系,拟定广州地区结核病传播链。培养结核分枝杆菌临床分离株并提取DNA,通过PCR扩增进行29位点MIRU-VNTR基因分型分析,并计算HGDI分辨力指数。以基因型成簇与否评价传播链内结核患者间传播关系的有无。结果:拟定了178条广州地区结核病传播链,复苏得到456株结核分枝杆菌临床分离株,提取获得其基因组DNA并扩增了29个VNTR位点,其组合HGDI分辨力指数为0.986。基因成簇性分析显示,有2条传播链内患者结核分枝杆菌临床分离株基因成簇,成簇率为1.10%。基因成簇性的2条传播链患者分别居住于同街道,发病时间间隔在2年以内,分别为9和14个月。结论:29位点MIRU-VNTR基因分型技术可用于广州地区结核分枝杆菌基因型分析,能否用于结核病传播关系需要进一步深入研究。

abstractsObjective:To explore the feasibility of MIRU-VNTR genotyping technology for analyzing the transmission of tuberculosis (TB) in Guangzhou.Methods:Based on the premise that TB patients lived in the same area, such as same villages and towns or the same community streets, might had transmission relations, the TB transmission chains in Guangzhou were drawn up. The clinical isolates of Mycobacterium tuberculosis were cultured, and the DNA was extracted. The 29-locus MIRU-VNTR genotyping analysis was performed by PCR amplification, and the HGDI resolution index was calculated. Genotype clustering was used to evaluate the presence or absence of transmission relationships among tuberculosis patients in the transmission chains.Results:178 TB transmission chains in Guangzhou were drawn up. 456 clinical isolates of Mycobacterium tuberculosis from resuscitated patients were amplified to obtain 29 VNTR sites, and the combined HGDI resolution index was 0.986. Gene clustering analysis showed that there were 2 transmission chains in which the genes of clinical isolates of Mycobacterium tuberculosis were clustered, with a clustering rate of 1.10%. The patients in the two transmission chains of gene clustering lived in the same street, and their onset time intervals was within 2 years, which were 9 months and 14 months, respectively.Conclusion:29-locus MIRU-VNTR genotyping technique can be used to analyze the genotype of Mycobacterium tuberculosis in Guangzhou, and whether it can be applied to the relationship between tuberculosis transmission needs further in-depth study.
