安徽省安庆市某三甲医院手足口病住院患儿流行病学特征及医疗费用研究
The epidemic characteristics and medical expense of hospitalized children with hand, foot, and mouth disease in a tertiary hospital of Anqing city, Anhui Provence
摘要目的 分析2014—2017年安徽省安庆市某三甲医院手足口病住院患儿的流行病学特征及住院医疗费用.方法 通过安庆市某三甲医院病案管理系统,收集2014—2017年该院年龄在18周岁以下的手足口病住院患儿的一般人口学信息、临床特征、治疗费用等进行分析.采用x2趋势检验比较不同年份重病手足口病患儿发生构成比的差异,采用两样本Mann-whitney秩和检验比较不同病情的手足口病患儿住院天数及住院费的差异.结果 2014—2017年某医院共收治手足口病患儿4151例,其中重症手足口病233例,死亡3例,各年重症比例呈下降趋势(P<0.01).手足口病发病时间分布主要集中在4月至7月及9月至11月;年龄以5岁以下为主,主要为散居儿童.手足口病住院患儿产生的医疗费用较高,中位住院费用为1604.2元/人.重症患儿的住院天数和住院费用均高于非重症患儿(P值均小于0.01).结论 安庆市4月至7月及9月至11月为手足口病高发月份,人口密集区流行强度大,5岁以下散居儿童为发病主要人群,且手足口病患儿产生的医疗费用较高.

abstractsObjective To analyze the epidemiological characteristics and medical expenses of hospitalized children with hand, foot, and mouth disease (HFMD) in a tertiary hospital of Anqing city, AnHui Provence, from 2014 to 2017. Methods The demographic information, clinical features and medical expenses of HFMD patients aged under 18 years old from 2014 to 2017 were collected through the medical record management system in a tertiary hospital in Anqing. Trend chi-square test was used to compare the constituent ratio of severe HFMD children among different years. Mann-whitney U test was used to compare the differences of hospitalization days and medical expense of HFMD children with various conditions. Results A total of 4151 cases of children with HFMD were reported in the tertiary hospital from 2014 to 2017, including 233 severe cases and 3 fatal cases. The annual ratios of the severe cases were decreasing (P<0.01). The peak periods of HFMD were from April to July and September to November. The HFMD patients were mainly scattered children under 5 years old. The medical expense of HFMD was high, with a median value of 1604.2 RMB per person. Both hospitalization days and medical expenses among severe cases were higher than that among mild cases (P<0.01). Conclusions The epidemic periods of HFMD in Anqing city were from April to July and from September to November, with high prevalence intensity in densely populated areas. Scatter children under 5 years old were mainly affected. In addition, the medical expenses incurred by HFMD cases were high.
