摘要目的 分析北京市通州区艾滋病筛查试验有反应样本抗体确证结果符合率,了解通州区各筛查实验室检测状况和不同试剂检测效力.方法 对2010-2016年通州区各级医疗机构艾滋病筛查实验室艾滋病筛查试验有反应样本2 072例进行蛋白免疫印迹法(western blot,WB)抗体确证试验,分析不同抗体筛查试剂的确证阳性符合率.采用SPSS 19.0进行描述流行病学统计分析.结果 通州区疾控中心、医院、中心血站的确证阳性符合率分别为90.20％、60.32％和18.25％;男性、女性确证阳性符合率分别为49.60％、11.23％;ELISA法筛查出的S/CO≥5的有反应样本中,确证阳性符合率为87.44％,高于1≤S/CO＜5的0.82％(P＜0.01).化学发光法S/CO≥10的确证阳性符合率81.86％高于1≤S/CO＜ 10的阳性符合率1.81％(P＜0.01).结论 HIV抗体筛查有反应样本的抗体确证阳性符合率与样本来源、性别以及S/CO值相关.抗体确证阴性和不确定样本建议开展核酸检测.

abstractsObjective To analyze the consistence rates of confirmatory tests and anti-HIV screening-reactive samples in Tongzhou district of Beijing,and to understand the testing status in the screening laboratories and detection capability of the reagents.Methods Western blot was used in confirmation tests for 2 072 anti-HIV screening-reactive samples from screening laboratories in the medical institutes in Tongzhou district during 2010-2016.Consistence rates of confirmed positive for different screening methods were analyzed.SPSS 19.0 software was applied for statistical analysis in the descriptive epidemiological analysis.Results The consistence rates of confirmed positive for samples from Tongzhou District Center for Disease Control and Prevention,hospitals and Tongzhou Blood Center were 90.20％,60.54％ and 18.25％,respectively.The consistence rates of confirmed positive for male and female cases were 49.60％ and 11.23％,respectively.For samples screened by ELISA,the consistence rate of confirmed positive for samples with S/CO≥5 was 87.44％ and was higher than the consistence (0.82％) for samples with S/CO＜5 (P＜0.01).For samples screened by chemoluminescence assay,the consistence rate of confirmed positive for samples with S/CO≥ 10 was 81.86％ and was higher than the consistence (1.81％) for samples with 1 ≤S/CO＜ 10 (x2=379.417,P＜0.01).Conclusions The consistence of confirmed positive for anti-HIV screening-reactive samples related to sample source,patients' gender and S/CO ratios.Nucleic acid testing is recommended for confirmed anti-HIV negative samples and uncertain samples.
