2017—2020年上海市流感相关流感样病例门急诊疾病负担估计
Disease burden of influenza-associated influenza-like illness in outpatients and emergency departments in Shanghai, 2017–2020
摘要目的:估计上海市2017—2020年流感相关门急诊负担,为流感疫苗的接种策略提供依据。方法:收集上海市2017—2020年30家流感监测哨点医院的门急诊流感样病例(influenza-like illness,ILI)数量和病原学检测结果,并且纳入气象因素,利用负二项回归模型估计上海市2017—2020流感相关ILI门急诊率。结果:2017—2020年,上海市30家全年开诊的哨点医院累计采集鼻咽拭子样本76 544份,流感病毒检出率为19.77%。上海市年均流感相关ILI门急诊数为274 475(212 099,400 461),由此估计的流感相关ILI门急诊率为12.05(9.27,17.65)/1 000人年。2017—2019流感门急诊负担呈现逐年上升的趋势,2020年因采取新冠综合疫情防控措施,流感的门急诊负担发生了显著的下降。2020年各亚型流感相关ILI门急诊率均低于2017—2019年平均水平。疾病负担最重流感亚型是B型流感6.62(5.08,9.64)/1 000人年,负担最重的年龄组的是儿童组(0~14岁)20.96(15.70,31.00)/1 000人年。结论:流感在上海引起了较高的医疗负担。2020年采取的新冠疫情综合防控措施可以降低流感的流行程度和减轻疾病负担。

abstractsObjective:To estimate the disease burden associated with influenza in the outpatient sections and emergency departments in Shanghai from 2017 to 2020, so as to provide evidences for influenza vaccination strategy.Methods:The number of influenza-like illness (ILI) cases and etiological detection results were collected from 30 sentinel hospitals for influenza surveillance in Shanghai from 2017 to 2020. The negative binomial regression model was fitted to estimate the influenza-associated excess ILI outpatient burden, taking into account of meteorological factor.Results:From 2017 to 2020, a total of 76 544 nasopharyngeal swab samples were collected from 30 sentinel hospitals, and the detection rate of influenza virus was 19.77%. Influenza was associated with an annual average number of 274 475 (212 099, 40 0461) ILI cases in outpatient sections and emergency departments. The estimated influenza-related ILI outpatient visiting rate was 12.05 (9.27, 17.65) per 1 000 person year. The disease burden of the outpatient sections and emergency departments of influenza showed a trend of increasing from 2017 to 2019, and decreased significantly in 2020 due to the implementation of comprehensive prevention and control measures. The disease burdens related to all subtypes of influenza in the were lower than the annual average levels in 2017-2019. The disease burden of influenza B virus (6.62 (5.08, 9.64) per 1 000 person year) was the highest. The age group with the highest burden was children (0-14 years old) (20.96 (15.70, 31.00) per 1 000 person year).Conclusions:Influenza caused high medical burden in Shanghai, and comprehensive prevention and control measures can reduce the prevalence of influenza and reduce the disease burden.
