北京市新型冠状病毒核酸检测混检策略应用
Application of pool testing strategy in SARS-Cov-2 nucleic acid detection in Beijing
摘要目的:通过收集北京市新型冠状病毒样本混采核酸检测结果,评价混采检出能力,总结规律,为新型冠状病毒及其它传染病病原体混采策略提供参考。方法:收集北京市2022年11月3日至17日10合1混采阳性样本,对其中核酸阳性感染者进行流行病学调查和实验室检测,汇总分析。结果:累计收集942管10合1混采阳性样本,每管检出(0.98±0.05)例感染者。混采开放读码框1ab(open reading frame,ORF1ab)基因、核衣壳蛋白(nucleocapsid protein,N)基因Ct值≥30占单采检出感染者812管混管的百分比分别为59.7%、67.9%。单采ORF1ab基因、N基因Ct值<30占检出923例感染者的百分比分别为72.2%、64.6%。混采与单采的O、N基因差值均值分别为(4.44±6.85)、(4.40±6.93)。空间分布显示,混管及单采阳性感染者分布存在明显空间聚集性。结论:混检策略在实际应用过程中能有效发现阳性感染者。今后开展人群感染情况检测时,应用混采筛查也能有效判断空间聚集性,实现预警作用。

abstractsObjective:To evaluate detection capability of pool testing and summarize characters of results by collecting the test results of pooled samples for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in Beijing, so as to provide reference for the pool testing strategy for detection of SARS-CoV-2 and other infectious disease pathogens in the future.Methods:The positive pooled samples from November 3 to 17, 2022 were collected. the epidemiological investigation and laboratory test were applied for nucleic acid positive persons and the results were summarize and analyzed.Results:A total of 942 "10-in-1" pooled samples with positive results were collected with (0.98 ± 0.05) nucleic acid positive person detected in each sample. In the 812 pooled samples with positive detection of patient by individual sampling, the proportion of positive in open reading frame 1ab (ORF1ab) gene and nucleocapsid protein (N) gene with Ct ≥ 30 were 59.7% and 67.9% respectively. Among 923 infected persons, the proportions of Ct <30 for ORF1ab and N gene by individual sampling were 72.2% and 64.6%, respectively. The mean differences for ORF1ab and N genes between pooled and individual samples were 4.44±6.85 and 4.40±6.93, respectively. The spatial distribution showed that there was obvious aggregation in the distribution of infected persons tested by pooled and individual sampling.Conclusions:The pool testing strategy can effectively detect positive persons in practice. Applying the strategy may also effectively identify the spatial clustering and realize the role of early warning in the future testing for population.
