摘要目的 探讨改良的3R护理法(reminiscencet往事回忆、realityorientations实物定位、remotivation记忆再激发)对老年痴呆并髋部骨折手术病人机体康复的影响.方法 对2005年6月至2007年12月轻中度痴呆老年髋部骨折手术的60例患者随机分成2组,实验组施行改良的3R护理法护理病人(在施行3R护理同时,加强病房环境管理、心理疏导,并结合图片、实物、多媒体演示等方法 指导患者行功能强化训练,帮助患者恢复及强化记忆,促进机体康复);对照组采用骨科手术病人常规护理.以上2组患者在实施护理前后均采用简易精神状态检查量表(MMSE)、日常生活活动量表(ADL)和社会适应功能障碍调查表(FAQ)对患者的精神状态、机体康复活动、髋关节功能、遵护嘱行为等进行评定,以确定临床护理效果.结果 实验组患者实施护理计划干预后MMSE、ADL、FAQ评分、遵护嘱行为及关节功能康复与对照组相比有显著性差异(P＜0.05,P＜0.01).结论 改良的个体化、综合性3R护理对老年痴呆髋部骨折手术患者的康复有明显促进作用,能改善AD骨折患者生活质量.

abstractsObjective To explore the influence of reforming 3R nursing process (reminiseencet, realityorientations, remotivation) to the patients rehabilition with senile dementia combining hip frature. Methods 60patients with senile dementia bombining hip frature were apart to two groups randomly. One group patients received reforming 3R nursing process (except 3R nursing process, combining the pictures, entity, media mix to reinforcement exercise, to help the patients restore and reinforce the memory), the control group patients received routine nursing process. Results All patients were evaluated with the MMSE, ADL and FAQ about mental state,rehabilition, hip function, obey the nursing order. The MMSE, ADL, FAQ, obey nursing order and hip function have significant difference between the two groups. Conclusion The reforming 3R nursing process have promotion to the patients with senile dementia combining hip frature, improve the quality of life.
