摘要目的:了解肿瘤重症患者失禁性皮炎的现状及影响因素。方法:选择2017年6月至2018年5月入住复旦大学附属肿瘤医院ICU的1 494例中发生失禁的191例患者为研究对象，其中发生轻度失禁性皮炎13例，中重度失禁性皮炎68例。采用自制调查表收集发生中重度失禁性皮炎患者的年龄、性别、诊断、营养评分、体重指数(BMI)、入住ICU的原因、PAT评分、Braden评分、愈合时间、失禁性皮炎的转归、原发病的转归、失禁的频率、失禁的类型、失禁的天数等因素，分析失禁性皮炎的发生与这些因素之间的相关性。结果:采用多元Logistic回归分析发现PAT评分、失禁的天数及失禁的频率是ICU失禁性皮炎的危险因素。结论:肿瘤重症患者中PAT评分、失禁天数及失禁频率是发生失禁性皮炎的影响因素，应采取针对性预防护理措施。

abstractsObjective:To understand the present situation and influencing factors of incontinence dermatitis in patients with severe tumor.Methods:A total of 191 patients with incontinence in 1 494 patients who were admitted to ICU from June 2017 to May 2018 were selected as the subjects, including 13 cases of mild incontinence dermatitis and 68 cases of moderate and severe incontinence dermatitis.A self-designed questionnaire was used to collect excessie incontinent dermatitis patients age, gender, diagnosis, nutrition score, BMI score, the reason of the ICU admission, PAT score, Braden grading, healing time, the outcome of incontinent dermatitis, primary disease outcome, the frequency of incontinence, the type of incontinence, incontinence factors such as the number of days.The correlation between the occurrence of incontinence dermatitis and these factors was analyzed.Results:The incidence of incontinence was 12.8%, the incidence of incontinence dermatitis in incontinence patients was 42.4%, and the incidence of pressure sores was 1.6%.The use of multiple Logistic regression analysis found that PAT score, the number of days of incontinence and the frequency of incontinence were the risk factors for incontinence dermatitis.Conclusions:PAT score, incontinence days and incontinence frequency are the influencing factors for the occurrence of incontinence dermatitis in patients with severe tumor, so specific preventive nursing measures should be taken for patients.
