三种术前阴道准备方法预防子宫切除术后医院感染的临床效果
Clinical evaluation of three methods of vaginal disinfection for prevention of nosocomial infection after hysterectomy
摘要目的:探讨三种术前阴道准备方法预防子宫切除术患者术后医院感染的临床效果评价。方法:选取乐山市人民医院2016年4月至2017年6月接受子宫切除术的患者180例,按照随机数字表法分为三组,其中A组58例为碘伏原液组,B组61例为臭氧组,C组61例为甲硝唑片+生理盐水组,均于术前1 d及术前当日擦洗阴道,对比三组患者术后体温(T)、白细胞计数(WBC)、中性粒细胞比例(N)、降钙素原(PCT)、超敏C反应蛋白(hs-CRP)及住院时间。结果:B组住院时间显著短于A组和C组,差异有统计学意义( P<0.01),术后第3天和第6天B组的WBC、N、hs-CRP和PCT低于A组和C组,差异有统计学意义( P<0.05),术后体温比较三组差异无统计学意义( P>0.05)。 结论:子宫切除术前用超声波臭氧妇科治疗仪进行阴道准备,可以降低患者术后WBC、N、hs-CRP和PCT指标,预防医院感染的发生,同时缩短住院时间提高床位周转率,具有临床应用价值。

abstractsObjective:To investigate the clinical effect of three vaginal preoperative vaginal preparation methods on preventing nosocomial infection after hysterectomy.Methods:A total of 180 patients who underwent hysterectomy from April 2016 to June 2017 were selected and divided into three groups according to the random number table method, among which 58 patients in group A were in the iodovate group, 61 patients in group B were in the ozone group, and 61 patients in group C were in the metronidazole tablets + normal saline group.All the patients in the three groups were swabbed on the day 1 and the day before surgery.Postoperative body temperature (T), white blood cell count (WBC), neutrophil ratio (N), procalcitonin progenitor (PCT), hypersensitive c-reactive protein (hs-CRP) and length of stay were compared among the three groups.Results:The length of stay of group B was significantly shorter than that of group A and group C, and the difference was statistically significant( P<0.01). The WBC, N, hs-CRP and PCT of group B were lower than those of group A and group C on the 3rd and 6th days after surgery, and the differences were statistically significant( P<0.05). There was no statistically significant difference in postoperative body T between the three groups ( P>0.05). Conclusions:Before hysterectomy, vaginal preparation with ultrasound ozone gynecologic therapy instrument can reduce WBC, N, hs-CRP and CT indexes after hysterectomy, prevent nosocomial infection, shorten hospitalization time and improve bed turnover rate.It has clinical application value.
