胰岛素不同给药方式对糖尿病酮症酸中毒患者病情的影响
Effects of different insulin administration methods on the condition of patients with diabetic ketoacidosis
摘要目的:探讨胰岛素不同给药方式对糖尿病酮症酸中毒(DKA)患者病情的影响。方法:选取2019年1月至2020年7月中山市人民医院内分泌科收治的DKA患者150例为研究对象,采用随机数字表法分为A组、B组和C组,各50例。在A组、B组和C组患者同时给予相同基础治疗的基础上,A组患者予以传统的小剂量静脉持续滴注胰岛素,B组予以小剂量微量泵静脉推注胰岛素,C组给予胰岛素泵持续皮下输注胰岛素治疗,观察并比较三组患者的血糖水平、治疗疗效以及各治疗相关指标。结果:三组胰岛素不同给药方式治疗后三组患者血糖水平变化比较差异无统计学意义( P>0.05);C组患者治疗疗效有效率显著高于A组、B组,C组患者的各治疗相关指标(血糖达标时间、血酮体转阴时间、尿酮体转阴时间)均明显优于A组、B组,差异均有统计学意义( P<0.05)。 结论:胰岛素泵持续皮下输注治疗可显著提升DKA患者的治疗疗效,有效地优化各项治疗相关指标,对患者获取更优质的治疗效果具有重要的作用。

abstractsObjective:To investigate the effects of different insulin administration methods on the condition of diabetic ketoacidosis (DKA) patients.Methods:A total of 150 patients with DKA admitted from January 2019 to July 2020 were selected as study subjects, and were divided into group A, Group B and group C by random number table method, with 50 patients in each group.On the basis of receiving the same basic treatment simultaneously in group A, group B and group C, patients in group A were given traditional low-dose intravenous infusion of insulin, patients in group B were given intravenous infusion of insulin with low-dose micropump, and patients in group C were given continuous subcutaneous insulin infusion by insulin pump.The blood glucose level, therapeutic effect and related indexes of the three groups were observed and compared.Results:There was no significant difference in the change of blood glucose level among the three groups by different administration methods of insulin ( P>0.05). The effective rate of treatment in group C was significantly higher than that in group A and B. The treatment related indicators of patients in group C (blood glucose normalization time, blood ketone body turning negative time, urine ketone body turning negative time) were significantly better than those in group A and Group B, and the differences were statistically significant( P<0.05). Conclusions:Insulin pump continuous subcutaneous infusion therapy can significantly improve the therapeutic efficacy of DKA patients, effectively optimize all treatment-related indicators, and play an important role in obtaining better therapeutic effect for patients.
