基于Go/Nogo任务的重度OSAHS患者认知功能研究
Cognitive study in patients with severe obstructive sleep apnea hypopnea syndrome based on Go/Nogo task
摘要目的 研究重度OSAHS患者认知功能的改变.方法 对25例重度OSAHS患者(重度OSAHS组)和年龄、性别、教育等方面与患者匹配的25名健康志愿者(正常对照组)进行基于Go/Nogo任务的行为学和事件相关电位分析.结果 重度OSAHS组患者年龄、性别、受教育时间、总睡眠时间、睡眠效率、REM期睡眠时间与正常对照组比较差异均无统计学意义.与正常对照组比较,重度OSAHS组体质量指数、N1期及N2期睡眠时间和呼吸暂停低通气指数升高,N3期睡眠时间减少,最低血氧饱和度降低,差异均有统计学意义(P值均<0.05).重度OSAHS组对Go刺激的反应正确率低于正常对照组(t =2.049,P<0.05);重度OSAHS组对Nogo刺激的正确率、反应时与正常对照组比较差异无统计学意义.重度OSAHS组FCz非靶N2波幅显著高于正常对照组(f=3.103,P<0.05),重度OSAHS组FCz靶N2波幅与正常对照组比较差异无统计学意义.结论 重度OSAHS患者的持续注意能力降低,冲突监测功能可能存在损害.基于Go/Nogo任务的行为学及事件相关电位测试有助于早期发现重度OSAHS患者的认知功能异常.

abstractsObjective To investigate behavior and electrophysiological manifestation of cognitive function in patients with severe obstructive sleep apnea hypopnea syndrome (OSAHS).Methods According to PSG results,25 severe OSAHS patients and 25 normal controls matched in age,sex,and education performed visual Go/Nogo task.Behavior and event related potential (ERP) measures were collected.Results There were no significant differences between severe OSAHS group and control group in age,sex,education,total sleep time,sleep efficiency and REM sleep time.Compared to control group,body mass index,N1 and N2 sleep time,and apnea hypopnea index were significantly increased,N3 sleep time and the lowest oxygen saturation were significantly decreased in severe OSAHS group (all P < 0.05).The accuracy of Go trials in severe OSAHS group was significantly lower than that in control group (t =2.049,P <0.05).There were no significant differences between severe OSAHS group and control group in the accuracy of Nogo trials and reaction time.The mean amplitude of Nogo-N2 at FCz site showed a significant difference between severe OSAHS group and control group (t =3.103,P <0.05).There were no significant differences between severe OSAHS group and control group in the amplitude of Go-N2 at FCz site.Conclusions Severe OSAHS patients have decreased ability of continuous attention and may have impaired conflict monitor function.Behavioral and ERP tests based on Go/Nogo task may help for early detection of abnormal cognitive function in patients with severe OSAHS.
